Background and Aim: The primary aim of this study was to evaluate the predictive value of cost-effective, easily applicable inflammatory biomarkers in forecasting the therapeutic response to intravenous (IV) and subcutaneous (SC) infliximab in patients with Crohn's disease. Materials and Methods: Patients aged 18–85 years who were followed at the Ege University Faculty of Medicine, Division of Gastroenterology, between April 2023 and December 2024 with a diagnosis of Crohn's disease, who had not previously received biological therapy, and who were initiated on infliximab due to active disease, were retrospectively analyzed. A total of 30 patients who completed induction therapy (weeks 0, 2, and 6) were included. Demographic characteristics; CDAI scores at weeks 0, 12, and 28; colonoscopy findings; biochemical and hematological parameters; and calculated inflammatory indices (NLR, PAR, LMR, MPVLR, SII, HALP, and PNI) were recorded. Clinical remission was defined as CDAI <150. Biochemical remission was defined as CRP <5 mg/L and fecal calprotectin <150 mg/kg. Endoscopic remission was defined as SES-CD 0–3, and for postoperative patients as Rutgeerts score i0–i1. The primary endpoint was achieving remission based on clinical, laboratory, or endoscopic assessment. Results: The median age of the study population was 42.5 years, and 43.3% were female; 46.7% had ileocolonic involvement and 23.3% had perianal disease. The clinical remission rate was found to be 78.3% at week 12 and 86.7% at week 28. Median CDAI decreased from 363 at baseline to 75 at week 12 and 86 at week 28 (p<0.001). CRP levels declined from 8.2 mg/L to 1.5 mg/L at week 12 and 2.0 mg/L at week 28 (p<0.001), while fecal calprotectin decreased from 200 μg/g to 61 μg/g at week 12 and 33.5 μg/g at week 28 (p<0.05). Significant improvements were observed in hematological inflammatory indices: NLR decreased from 3.19 to 1.94, PLR from 224.5 to 121.3, PAR from 8.40 to 6.28, and SII from 1133 to 603 (all p<0.001), while LMR increased from 2.66 to 3.88 (p=0.04). Nutritional markers also improved: HALP increased from 21.8 to 51.7 and PNI from 48.2 to 55.3 (p<0.001). Patients achieving early remission had shorter disease duration (3 vs. 16 years; p=0.03), lower NLR and SII (p<0.05), and higher LMR levels (p=0.04). Endoscopic remission was achieved in 80% of patients at week 12; all patients without endoscopic remission were receiving steroids, which was statistically significant (p=0.02). Conclusion: Infliximab therapy provides high rates of early clinical, biochemical, and endoscopic remission in patients with Crohn's disease. The marked improvement in inflammatory indices following treatment suggests that hematological biomarkers—particularly LMR, NLR, and SII—serve as strong and practical predictors of remission. The increase in nutritional indices further reflects therapeutic success through reduced systemic inflammation. The association between steroid use and lack of endoscopic remission suggests a more refractory inflammatory phenotype. Overall, these findings support the utility of hematological indices as accessible, cost-effective, and noninvasive biomarkers in assessing treatment response in Crohn's disease; however, validation in larger, prospective cohorts is required.
