Objective: This study aims to evaluate the survival rates and clinical performance of different flowable composite materials applied to non-carious cervical lesions. Materials and Methods: The study included an analysis of 214 restorations performed on 65 patients (41 females and 24 males) treated at the Department of Restorative Dentistry, Faculty of Dentistry, Ege University. Three different flowable composite materials were assessed: Ruby Flow, Estelite Universal Flow and Nova Compo SF. In Estelite Universal Flow and Ruby Flow restorations, Optibond Universal adhesive agent was applied following selective etching. Since Nova Compo SF has self adhesive properties, it was applied in three different ways: direct application, selective etching before application, or selective etching followed by Optibond adhesive. The restorations were evaluated at 1, 3, and 6 months using the Modified USPHS criteria. Statistical analyses were performed using Pearson’s Chi-Square test, Fisher’s Exact test, and Cochran’s Q test, with a significance level set at p<0.05. Results: Statistically significant differences were observed in retention and marginal adaptation among the different flowable composite materials at the 1st, 3rd, and 6th months of evaluation (p<0.05). Restorations in the Ruby Flow, Estelite Universal Flow and Adhesive+Nova Compo SF groups exhibited better retention and marginal adaptation compared to the Nova Compo SF group. At the end of the six month period, Nova Compo SF exhibited the lowest retention rate at %2,3. Additionally, the retention scores of the Etch+Nova Compo SF, Vertise Flow, and Etch+Vertise Flow groups showed a decreasing trend over time (p<0.05). Conclusion: In this study, the clinical performance of different flowable composite materials was compared, and self-adhesive flowable composites showed lower retention rates when used without acid etching and adhesive application. Marginal adaptation and color stability were found to be superior in groups where an adhesive system was applied. The results indicate that, in the restoration of non-carious cervical lesions, the bonding mechanism and long-term success of the composite material used should be carefully considered. Particularly in cases where flowable composites are utilized, the use of an adhesive system prior to application is essential, regardless of the type of composite material.
