Özet
Araştırma hemodiyaliz hastalarına verilen "Kolcaba'nın Konfor Kuramı'na göre yapılandırılmış hemşirelik bakımının" semptom şiddeti ve konfora etkisini değerlendirmek amacıyla yapılmıştır. Araştırma yarı deneysel olarak Kasım 2021- Mayıs 2022 tarihleri arasında Karaman Eğitim ve Araştırma Hastanesi Hemodiyaliz Ünitesi'nde yürütülmüştür. Araştırmanın örneklemini, uygulama grubu 22, kontrol grubu 24 olmak üzere toplam 46 hemodiyaliz hastası oluşturmuştur. Araştırmanın verileri Hasta Tanıtım Formu, Diyaliz Semptom İndeksi ve Hemodiyaliz Konfor Ölçeği- Versiyon II kullanılarak toplanmıştır. Uygulama grubunda yer alan hastalara 2 hafta boyunca "Kolcaba'nın Konfor Kuramı'na göre yapılandırılmış hemşirelik bakımı" araştırmacı tarafından verilmiştir. Ayrıca bakımı desteklemek amacıyla araştırmacılar tarafından ilgili literatür incelenerek hemodiyaliz hastalarında görülen fiziksel, psikospiritüel, çevresel ve sosyokültürel sorunlar dikkate alınarak hazırlanan "Kolcaba'nın konfor kuramına göre yapılandırılmış hemodiyaliz hastalarının bakımına yönelik eğitim kitapçığı" hastalara yazılı doküman olarak verilmiştir. Araştırmanın uygulanabilmesi için etik izin ve hastane yazılı kurum izni alınmıştır. Uygulama grubunun 1. gün, 15.gün ve 21. gün Diyaliz Semptom İndeksi puan ortalamaları sırasıyla; 46.90±17.70, 34.81±14.00 ve 30.59±12.31 olarak belirlenmiştir. Uygulama grubundaki hastaların Diyaliz Semptom İndeksi puan ortalamalarının zamana göre azaldığı saptanmıştır. Kontrol grubunun 1. gün, 15.gün ve 21. gün Diyaliz Semptom İndeksi puan ortalamaları sırasıyla; 48.66±15.26, 50.87±16.03 ve 51.12±15.36 olarak belirlenmiştir. Kontrol grubundaki hastaların Diyaliz Semptom İndeksi puan ortalamalarının ise zamana göre arttığı belirlenmiştir. Uygulama grubunun 1. gün, 15.gün ve son test Hemodiyaliz Konfor Ölçeği- Versiyon II toplam puan ortalamaları sırasıyla; 94.18±9.57, 101.90±8.00 ve 102.81±7.70 olarak belirlenmiştir. Uygulama grubundaki hastaların Hemodiyaliz Konfor Ölçeği- Versiyon II toplam puan ortalamalarının zamana göre arttığı saptanmıştır. Kontrol grubunun 1. gün, 15.gün ve 21. gün Hemodiyaliz Konfor Ölçeği- Versiyon II toplam puan ortalamaları sırasıyla; 93.75±14.01, 92.25±13.50 ve 92.45±13.62 olarak belirlenmiştir. Kontrol grubundaki hastaların Hemodiyaliz Konfor Ölçeği- Versiyon II toplam puan ortalamalarının zamana göre azaldığı saptanmıştır. "Kolcaba'nın Konfor Kuramı'na göre yapılandırılmış hemşirelik bakımının" hemodiyaliz hastalarının diyalize bağlı semptomlarını azaltmakta ve konforlarını arttırmaktadır. Bu araştırma sonuçları, hemodiyaliz hastalarının hemşirelik bakımında hastayı bütüncül olarak ele alan hemşirelik kuram ve modellerinin kullanılmasının önemini ortaya koymuştur
The research was conducted to evaluate "The effect of nursing care structured according to Kolcaba's Comfort Theory", which is given to hemodialysis patients, on symptom severity and comfort. The research was carried out semi-experimentally between November 2021 and May 2022 in Karaman in Turkey Training and Research Hospital Hemodialysis Unit. The sample of the study consisted of 46 hemodialysis patients, 22 in the application group and 24 in the control group. The data of the study were collected using the Patient Diagnosis Form, Dialysis Symptom Index, and Hemodialysis Comfort Scale-Version II. Nursing care structured according to Kolcaba's Comfort Theory was given to the patients in the application group for 2 weeks by the researcher. In addition, in order to support care, the related literature was reviewed by the researchers, and the "educational booklet for the care of hemodialysis patients, structured according to Kolcaba's comfort theory", which was prepared considering the physical, psychospiritual, environmental and sociocultural problems in hemodialysis patients, was given to the patients as a written document. Ethical approval and hospital institution permission were obtained for the implementation of the study. The 1st day, 15th day and 21st day Dialysis Symptom Index score averages of the treatment group were respectively; It was determined as 46.90±17.70, 34.81±14.00 and 30.59±12.31. It was determined that the Dialysis Symptom Index score averages of the patients in the treatment group decreased over time. Dialysis Symptom Index score averages of the control group on the 1st day, 15th day and 21st day, respectively; It was determined as 48.66±15.26, 50.87±16.03 and 51.12±15.36.
