Özet
Amaç: Araştırma, Tidal Model'e dayalı psikiyatri hemşireliği yaklaşımının, etkisiz bireysel baş etme ve benlik kavramında bozulma tanısı alan alkol bağımlılarında etkisini belirlemek amacıyla yapılmıştır. Yöntem: Ön test-son test değerlendirmeli, kontrol gruplu ve yarı deneysel nitelikte düzenlenen bu araştırma, Ekim 2013 ve Aralık 2014 tarihleri arasında, İzmir Katip Çelebi Üniversitesi Atatürk Eğitim ve Araştırma Hastanesi AMATEM Kliniği'nde yürütülmüştür. AMATEM kliniğinde yatarak tedavi gören alkol bağımlıları araştırmanın evrenini, belirlenen kriterleri karşılayan ve araştırmaya katılmayı kabul eden alkol bağımlıları araştırmanın örneklemini oluşturmuştur. Örneklem büyüklüğü power analiz yapılarak bulunmuş, deney ve kontrol grubuna 18'er bireyin alınmasına karar verilmiştir. Araştırmada veriler, Birey Tanıtım Formu, Bağımlılık Profil İndeksi (BAPİ), Tedavi Motivasyonu Anketi (TMA), Başa Çıkma Tutumlarını Değerlendirme Ölçeği (COPE), Coopersmith Benlik Saygısı Envanteri (BSÖ), Beck Depresyon Envanteri (BDE) ve Beck Anksiyete Envanteri (BAE) kullanılarak toplanmıştır. Deney grubundaki bireylere bireyselleştirilmiş bakım sağlamak için, bireysel görüşmeler yapılmış ve bireylerin hedefleri ve araştırmanın hedefleri doğrultusunda girişimler uygulanmıştır. Bulgular: Deney grubundaki bireylerin yaş ortalaması 43.33±6.55, kontrol grubundaki bireylerin yaş ortalaması 42.44±7.03'tür. Her iki gruptaki bireylerin %11.1'i kadın, %88.9'u erkek'tir. Her iki gruptaki bireylerin çoğunluğunun evli yada boşanmış, ilkokul mezunu olduğu, bir işte çalıştığı saptanmıştır. Görüşmeler öncesi, deney ve kontrol grubu bireylerin COPE alt ölçek puanları ve BSÖ puan ortalamaları arasında istatistiksel olarak anlamlı bir fark bulunmamıştır (p>0,05). Görüşmeler sonrası, deney ve kontrol grubu bireylerin COPE alt ölçekleri olan "Pozitif yeniden yorumlama ve gelişme", "Yararlı sosyal destek kullanımı", "Aktif başa çıkma", "Geri durma", "Duygusal sosyal destek kullanımı" ve "Madde kullanımı" puan ortalamaları arasında istatistiksel olarak anlamlı fark bulunmuştur (p<0.05). Görüşmeler sonrası, deney grubu bireylerin BSÖ puanının, kontrol grubuna göre yüksek olduğu; fakat, aradaki farkın anlamlı olmadığı saptanmıştır (p>0.05). Görüşmeler sonrasında, deney grubu bireylerin Pozitif Yeniden Yorumlama ve Gelişme, Yararlı Sosyal Destek Kullanımı, Aktif Başa Çıkma, Geri Durma, Plan Yapma, Davranışsal Olarak Boş Verme ve Madde Kullanımı alt ölçek puanlarında ve BSÖ puanında, ön test puanlarına göre, olumlu yönde değişiklik oluştuğu bulunmuştur. Sonuç: Sonuç olarak, Tidal Model'e dayalı psikiyatri hemşireliği yaklaşımının, alkol bağımlılarının Pozitif Yeniden Yorumlama ve Gelişme, Aktif Başa Çıkma, Geri Durma, Duygusal Sosyal Destek Kullanımı ve Plan Yapma puanlarını artırmak, Davranışsal Olarak Boş Verme puanını ise düşürmek için etkin bir yaklaşım olduğu ortaya konmuş; benlik saygısını arttırmada ise anlamlı bir etkiye sahip olmadığı belirlenmiştir
Objectives: The study was designed and applied for assessing the effects of psychiatric nursing approach based Tidal Model on alcohol-dependent persons with nursing diagnosis of ineffective coping and disturbed self-concept. Methods: The study was a test-retest controlled and quasi-experimental design. The study was performed in İzmir Katip Çelebi University, Atatürk Education and Research Hospital, AMATEM clinic, from October 2013-December 2014. The universe of the study was formed with alcohol-dependent persons who were inpatient in AMATEM clinic. The sample of the study was formed with alcohol-dependent persons according to criteria of study and who accepted the study. The sample size was found to be using power analysis. The study was conducted with 18 in study-group and 18 in control-group. The data was collected with the Personal Definition Form, The Addiction Profile Index, The Treatment Motivation Questioneer, The Coping Inventory, Coopersmith Self-Esteem Inventory, Beck Depression Inventory and Beck Anxiety Inventory. Individual sessions were made to provide individualized care to alcohol-dependent persons in study-group and interventions were implemented for aims of study and individual goals. Results: The mean age of the study group was 43.33±6.55 and the mean age of the control group was 42.44±7.03. In two groups, 11.1% of persons were females, 88.9% of persons were males. In both groups, most of them were married and divorced and had graduated from primary school. Most of the participants were worked at a job. Before sessions, there were no significant differences between two groups on The Coping Inventory and Coopersmith Self-Esteem Inventory. After sessions, there were significant differences between two groups on Positive reinterpretation and growth, Use of instrumental social support, Active coping, Restraint coping, Use of emotional social support and Alcohol-drug disengagement. After sessions, the mean of Coopersmith Self-Esteem Inventory in study group were high than control group but, there were no significant differences between two groups. After sessions, in study group, positive differences were found on Positive reinterpretation and growth, Use of instrumental social support, Active coping, Restraint coping, Planning, Behavioral disengagement, Alcohol-drug disengagement and Self-Esteem. Conclusion: The results of this study indicate that an psychiatric nursing approach based Tidal Model may have a positive impact for alcohol-dependent persons with nursing diagnosis of ineffective coping and disturbed self-concept on Positive reinterpretation and growth, Active coping, Behavioral disengagement, Restraint coping, Seeking of emotional social support, Planning, and may not increase the level of Self-Esteem.
