Özet
Aktinik keilit invaziv skuamöz hücreli karsinomaya dönüşebilen prekanseröz bir lezyondur. Fotodinamik terapi, hücre içinde biriken ışığa duyarlandırıcı maddelerin sitotoksik fotoaktivasyonu prensibine dayanan bir tedavi modelidir. Bu çalışmanın amacı aktinik keilit tedavisinde fotodinamik terapinin etkinliğini değerlendirmekti. Aktinik keilitli sekiz hasta 5-aminolevülinik asitin (ALA) yerel uygulamasını takiben inkoherent ışık kaynağı ile ışınlanarak başarı ile tedavi edilmiştir. Işınlama sırasında 580-720 nm dalga boyunda kırmızı ışık kullanılmıştır. Tedavi sırasında tüm hastalar yanma hissi ile hafif ağrı bildirmişlerdir. Tedavi öncesi ve tedaviden 2 ay sonra klinik ve histolojik değerlendirme yapılmıştır. Klinik değerlendirmede altı hasta (%75) tam düzelme gösterirken, histopatolojik değerlendirmede 5 hastada (%62.5) tam düzelme oranı saptanmıştır. on iki aylık takip süresince herhangi bir rekürrens bulgusu saptanmamıştır. Sonuç olarak ALA ile kombine fotodinamik tedavinin aktinik keilit tedavisinde etkin bir tedavi modeli olduğu sonucuna varılmıştır.
Bacground and design: Actinic cheilitis is a precancerous lesion, which may undergo malignant transformation into invasive squamous cell carcinoma. Photodynamic therapy (PDT) is a treatment modality based on the cytotoxic photoactivation of photosensitizers accumulated in cells. the purpose of this study was to determine the effectiveness of photodynamic therapy in the treatment of actinic cheilitis. Materials and methods: Eight patients with actinic cheilitis were succesfully treated by photodynamic therapy using the topical application of 5-aminolevulinic acid (ALA), followed by irradiation with incoherent visible light. the red light was used during irradiation in the wavelength of 580-720 nm. During photodynamic therapy, all patients reported a burning sensation and moderate pain. Histological and clinical evaluation were performed both before treatment and two months after irradiation. Results: in clinical evaluation,complete improvement was achieved in six patients (75%). Histopathologically, five patients (62.5%) showed complete improvement rate. Follow-up (12 months) has no sign of recurrence. Conclusion: As a result, it was concluded that topical photodynamic therapy combined with ALA is an effective modality in the treatment of actinic cheilitis
