Özet
Bu araştırma; sindirim sistemi kanseri tanısıyla cerrahi girişim geçirmiş hastalarda kemoterapinin (5-Florourosil) neden olduğu OM'in önlenmesinde karadut şurubunun ve Hyalüronik asitin (Gelclair) etkinliğini değerlendirmek amacıyla planlanmış randomize, kontrollü deneysel bir çalışmadır. Araştırma öncesi gerekli izinler alındı. Araştırmanın yürütülmesi esnasında Ege Üniversitesi Bilimsel Araştırma Projeleri (BAP) Koordinatörlüğü'nce finansal destek sağlandı. Araştırma Muğla'da bir Eğitim Araştırma Hastanesinin Kemoterapi Ünitesi ve Onkoloji servisinde 22 Ocak-29 Ağustos 2019 tarihleri arasında yapılmıştır. Hasta sayısını belirlemek için power analizi yapıldı. Birinci gruba karadut şurubu (36 hasta), ikinci gruba (36 hasta) Hyalüronik asit (Gelclair) ve üçüncü gruba kliniğin standart protokolü (34 hasta) uygulandı. Veriler bireylerin sosyo-demografik ve hastalığa ilişkin özelliklerini içeren soru formu, DSÖ'nün Mukozit Evreleme Ölçeği, Orofaringeal Mukozitli Hastalarda Yaşam Kalitesi Ölçeği (Life Quality Scale in Patients with Oropharyngeal Mucositis–QMQoL) ve hastaların kullandıkları solüsyonlardan memnuniyetini değerlendirmek için memnuniyet değerlendirme skalası ile toplanmıştır. Araştırmacı tarafından her üç gruptaki hastalar gözlem süresinin 7., 14. ve 21. gününde değerlendirilerek Mukozit Evreleme Ölçeği ve Yaşam Kalitesi Ölçeği puanları kaydedildi. Verilerin değerlendirilmesinde frekans ve ortalama ± standart sapma, Ki Kare analizi, Kruskal Wallis, Post Hoc testlerden Bonferroni ikili karşılaştırma testi kullanılmıştır. Grupların 7., 14. ve 21. günlerde hiçbir grupta evre 3 ve 4 mukozite rastlanmamıştır. 7. günde Evre 0, 1 ve 2 açısından istatistiksel olarak anlamlı bir ilişki olduğu 1. Grupta sadece evre 1 mukozit geliştiği saptanmıştır. 14. günde; üçüncü Grupta evre 1 mukozit %8,8 oranında görülürken, bu oranın ikinci Grupta %5.6 iken, birinci Grupta mukozite rastlanmamıştır. 21. günde Evre 0, 1 ve 2 açısından istatistiksel olarak anlamlı bir ilişki olduğu 1. Grupta sadece evre 1 mukozit geliştiği, 3. Grubun %32'sinde evre 1, 2. Grubun %22.2'sinde ve 1. grubun %13.9'unda evre 1 mukozit geliştiği belirlendi. Araştırmanın bulguları doğrultusunda kemoterapi alan hastalarda ağız bakımı için karadut şurubu kullanımının oral mukozit sıklığını ve şiddetini azalttığı saptanmıştır.
This study is a randomized, controlled experimental study designed to evaluate the efficacy of black mulberry syrup and hyaluronic acid (Gelclair) in the prevention of CT-induced OM (5-Florourosil) in patients undergoing surgery for the diagnosis of gastrointestinal cancer. Before study, permission was received. Financial support was provided by Ege University Scientific Research Projects (BAP) Coordinator. The research was performed in a chemotherapy unit and oncology service of a Training and Research Hospital in Muğla on 22 January-29 August. Power analysis was made in order to determine the number of patients. Black mulberry syrup were administered (36 patients) to the first group; hyaluronic acid (Gelclair) were administered (36 patients) to the second group, and the standard protocol of the clinic were administered (34 patients) to the third group. The data is structured questionnaire including socio-demographic and disease characteristics of the individuals, WHO's mucositis staging scale, WHO Quality of Life in Patients with Oropharyngeal Mucositis (QMQoL) and the satisfaction of the patients with the solutions to evaluate satisfaction. collected with. The patients were evaluated by the observer on the 7th and 14th and 21st days by the researcher and the scores of mucositis staging and quality of life scale were recorded. Frequency and mean ± standard deviation, Chi-square analysis, Kruskal Wallis and Bonferroni paired comparison tests were used to evaluate the data. Stage 3 and 4 mucositis were not observed in all groups on days 7, 14 and 21. On day 7, there was a statistically significant relationship between Stage 0, 1 and 2 and only stage 1 mucositis developed in group 1. ; In the third group, stage 1 mucositis was observed in 8.8% of the patients, whereas in the second group, 5.6% had no mucositis in the first group. On day 21, there was a statistically significant relationship between stage 0, 1 and 2, stage 1 mucositis, stage 1, stage 1, stage 1 in 32%, stage 2, stage 2 and stage 2 in 22.2%. mucositis was determined. According to the findings of the study, it was found that the use of black mulberry syrup for oral care in patients receiving chemotherapy reduced the frequency and severity of oral mucositis.
