| dc.description.abstract | Amaç: Bu araştırma; Yoğun Bakım Hemşirelerinin Tükenmişlik Seviyesinin Deliryumlu Hastalara Bakım Vermede Karşılaşılan Güçlüklere Etkisinin incelenmesi amacıyla yapılmıştır. Yöntem: Yoğun bakım hemşirelerinin tükenmişlik seviyesinin deliryumlu hastalara bakım vermede karşılaşılan güçlüklere etkisinin incelendiği tanımlayıcı tipteki bu çalışmada karma araştırma yöntemi kullanılmıştır. Araştırmanın 1. Aşamasında örneklemini 120 yoğun bakım hemşiresinin oluşturduğu kişilerin sosyo-demografik verileri ve maslach tükenmişlik ölçeği kullanılarak kişilerin tükenmişlik düzeyleri belirlenmiştir. Araştırmanın 2. aşamasında ise araştırmanın 1. aşamasına katılan yoğun bakım ünitesinde çalışan hemşirelerden maslach tükenmişlik düzeyi yüksek, orta ve düşük 20 kişi ile yarı yapılandırılmış görüşme formu kullanılarak deliryumlu olan hasta bakımında karşılaştığı güçlükleri belirlemek amacıyla yapılmıştır. Bulgular: Araştırma sonucunda yoğun bakım hemşirelerinin ortalama 26.14±3.70 yaşında olduğu, yoğun bakım ünitesinde ortalama 7,04±2,23 yıl çalıştığı, %87,5'i lisans mezunu, Deliryum ile ilgili eğitim alma durumları incelendiğinde %77,5'i evet yanıtını vermiştir. Yoğun bakımda deliryumda olan hasta ile karşılaşma durumu incelendiğinde %73,3'ü çok sık yanıtını vermiştir. Araştırmaya katılan yoğun bakım hemşirelerinin Maslach Tükenmişlik Ölçeği Alt Grupları Puan Ortalaması incelendiğinde; duyarsızlaşma puan ortalaması 7,8917 orta düzey, duygusal tükenme puan ortalaması 17,2333 orta düzey, kişisel başarı puanı 21,7167 yüksek olarak bulunmuştur. Hemşirelerin deliryum yönetiminde yaşadıkları güçlükler/ zorluklar incelendiğinde; yaralanma riski, hastayı anlama zorluğu, uyku problemi olan hasta, tükenmişlik, deneyimsizlik, hastanın demans tanısının olması ve baş etme güçlüğüdür. Bu zorluklarla baş etmek için hemşireler; fedakârlık, sabırlı olmak ve destek almak gibi yöntemleri kullanmışlardır. Sonuç: Bu araştırmada; tükenmişlik alt boyutlarından duyarsızlaşma orta düzeyde, duyusal tükenme orta düzeyde ve kişisel başarıda azalma yüksek düzeyde bulunmuştur. Hemşirelerin deliryum yönetiminde yaşadıkları güçlükler/ zorluklar incelendiğinde; yaralanma riski, hastayı anlama zorluğu, uyku problemi olan hasta, tükenmişlik, deneyimsizlik, hastanın demans tanısının olması ve baş etme güçlüğüdür. | dc.description.abstract | Purpose: This research; It was conducted to examine the Effect of Burnout Level of Intensive Care Nurses on the Difficulties Encountered in Caring for Patients with Delirium. Method: The mixed research method was used in this descriptive study, which examined the effects of the burnout level of intensive care nurses on the difficulties encountered in providing care to patients with delirium. In the first stage of the study, the burnout levels of the individuals were determined by using the socio-demographic data of the individuals whose sample consisted of 120 intensive care nurses and the maslach burnout scale. In the second stage of the research, it was conducted to determine the difficulties faced by the patient with delirium by using a semi-structured interview form with 20 nurses working in the intensive care unit, who participated in the first stage of the research, with 20 people with high, medium and low maslach burnout levels. Results: As a result of the research, the average age of intensive care nurses was 26.14±3.70 years, they worked in the intensive care unit for an average of 7.04±2.23 years, 87.5% of them had undergraduate degrees, and when their educational status about delirium was examined, 77.5% he answered yes. When the situation of encountering the patient with delirium in the intensive care unit was examined, 73.3% of them gave the answer very often. When the Maslach Burnout Scale Subgroup Scores of the intensive care nurses participating in the research were examined; The mean depersonalization score was 7.8917 moderate, the emotional exhaustion mean score was 17.2333 moderate, and the personal achievement score was 21.7167 high. When the difficulties/difficulties experienced by nurses in delirium management are examined; risk of injury, difficulty in understanding the patient, the patient with sleep problems, burnout, inexperience, the patient's diagnosis of dementia, and coping difficulties. To cope with these difficulties, nurses; They used methods such as self-sacrifice, patience and getting support. Conclusion: In this study; Among the sub-dimensions of burnout, depersonalization was found at medium level, sensory exhaustion was at medium level, and the decrease in personal achievement was at high level. When the difficulties/difficulties experienced by nurses in delirium management are examined; risk of injury, difficulty in understanding the patient, the patient with sleep problems, burnout, inexperience, the patient's diagnosis of dementia, and coping difficulties.
