8-36 ay arası işitme kayıplı çocukların iletişim becerilerinin değerlendirilmesi

Özet
Amaç:Bu çalışmada anadili Türkçe olan 8-36 ay arası işitme kayıplı çocukların iletişim becerilerinin değerlendirilmesi amaçlanmıştır. Yöntem:Bu çalışmaya, Ege Üniversitesi Tıp Fakültesi Kulak Burun Boğaz (KBB) Anabilim Dalı'nda, KBB kliniğine ilk kez başvuran ya da takip edilen 8-36 ay yaş aralığındaki işitme kayıplı (İK) çocuklar ile 8-36 ay yaş aralığındaki normal işiten (Nİ) çocuklar dahil edilmiştir. Araştırma grubunu oluşturan 8-36 ay aralığındaki çocukların, iletişim ve dil becerilerini, jestler ve hareketler, sözcük dağarcığı (SD) ile dilbilgisi alanlarını değerlendirmek amacıyla Türkçe İletişim Gelişim Envanteri (TİGE) kullanılmıştır. TİGE, MacArthur-Bates Communicative Development Inventory (MacArthur-Bates İletişim Gelişim Envanteri /MB-CDI)'nden Ayhan AksuKoç ve arkadaşları tarafından 2019 yılında Türkçeye uyarlanmıştır. TİGE envanteri, TİGE-I ve TİGE-II'den oluşmaktadır. TİGE-I, 8-16 ay arası dil gelişim özelliklerini değerlendirirken TİGE-II 16-36 ay arası dil gelişim özelliklerini değerlendirmektedir. Bu çalışamada nedensel-karşılaştırmalı araştırma modeli kullanılmıştır. Araştırma grubunda yer alan bütün katılımcılara TİGE uygulanmış olup test sonuçları Statistical Package for the Social Sciences (SPSS) programı ile analiz edilmiştir. Bulgular:Sonuçlara göre 8-36 ay yaş aralığındaki İK'lı çocukların iletişim becerilerinde cinsiyete göre yapılan karşılaştırmalarda anlamlı farklılık olmadığı bulunmuştur.8-16 ay yaş aralığındaki Kİ ve Nİ çocukların TİGE I erken sözcükler, alıcı ve ifade edici sözcük dağarcığı, hareketler ve jestler (erken jestler ve geç jestler) puanları karşılaştırılmış ve Nİ çocukların lehine anlamlı farklılık olduğu bulunmuştur. 16-36 ay aralığındaki Kİ ve Nİ çocukların TİGE II ifade edici sözcük dağarcığı, dilbilgisel yapıları kullanmadan önce sözcükleri kullanarak geçmişteki olaylara, gelecekteki olaylara, o anda orada bulunmayan kişiler veya nesnelere atıfta bulunup bulunmadığı ve cümleler ve dilbilgisi bölümüne ait puanlarının tümünde Nİ çocukların lehine anlamlı farklılık olduğu bulunmuştur.Elde edilen sonuçlara göre 8-36 ay yaş aralığındaki İK'lı çocukların iletişim becerilerinde cinsiyete özgü farklılıklara rastlanmamıştır. 8-16 ay aralığındaki Nİ ve İK'lı çocukların TİGE-I erken sözcükler, alıcı ve ifade edici sözcük dağarcığı, hareketler ve jestler alt testlerinde ve 16-36 ay aralığındaki Nİ ve İK'lı çocukların ifade edici sözcük dağarcığı, dilbilgisel yapıları kullanmadan önce sözcükleri kullanarak geçmişteki olaylara, gelecekteki olaylara, o anda orada bulunmayan kişiler veya nesnelere atıfta bulunup bulunmadığı ve cümleler ve dilbilgisi alt testlerinde istatistiksel olarak anlamlı farklar bulunmuştur. Sonuç:Sonuçlara göre 8-36 ay aralığındaki İK'lı çocukların iletişim becerilerine ilişkin ortalama puanlarının Nİ çocuklara göre daha düşük olduğu saptanmıştır. İşitme kayıplı çocukların erken dönemdeki iletişim becerilerinin ayrıntılı olarak değerlendirilmesi ve elde edilen sonuçlara göre çocukların terapi ve eğitim programlarına oluşturulması, ileri dönmede çocukların dil, konuşma ve akademik başarılarına katkı sağlayacaktır. Aim:In this study, it was aimed to evaluate the communication skills of children with hearing loss aged 8-36 months whose native language is Turkish. Method:This study was carried out by including the ages of 8-36 months children with normal hearing (NH) and children with HL between the ages of 8-36 months who applied to the ENT clinic for the first time or were followed up in Ege UniversityHospital, Department of Ear Nose and Throat (ENT). The Turkish Communication Development Inventory (TİGE) was used to evaluate the communication and language skills of the children aged 8-36 months, who make up the research group, in (1) gestures and movements, (2) vocabulary, and (3) grammar. It was adapted by Ayhan Aksu - Koç et al. from TİGE, MacArthur-Bates Communicative Development Inventory (MacArthur-Bates Communication Development Inventory /MB-CDI) in 2019. The adaptation TİGE inventory consists of TİGE-I and TİGE-II. While TİGE-I evaluates language development characteristics between 8-16 months, TİGE-II evaluates language development characteristics between 16-36 months. In this study, a causal-comparative research model was used. TİGE was applied to all participants in the research group and the test results were analyzed using the Statistical Package for the Social Sciences (SPSS). Results:According to the results, it was found that there was no significant difference in the communication skills of children with hearing loss (HL) between the ages of 8-36 months in comparisons made by gender. TIGE I early words, receptive and expressive vocabulary, movements and gestures (early gestures and late gestures) scores of HL and normal hearing (NH) children aged 8-16 months were compared and it was found that there was a significant difference in favor of NH children.When the TIGE II results of HL and NH children aged 16-36 months are compared, expressive vocabulary, whether they refer to past events, future events, people or objects that are not there at the time, by using words before using grammatical structures, and in all of the sentences and grammar section scores showed that there was a significant difference in favor of NH children.According to the results obtained, no gender-specific differences were found in the communication skills of children with HL between the ages of 8-36 months. Statistically significant differences were found in the TIGE-I early words, receptive and expressive vocabulary, movements and gestures subtests of 8-16 months old children with NH and HL. Statistically significant differences were found in the expressive vocabulary, sentences and grammar subtests and whether the 16-36 month-old children with NH and HL referred to past events, future events, people or objects that were not there at the time, using words before using grammatical structures. Conclusion:According to these results, it was determined that the average scores of communication skills of children with HL between 8-36 months were lower than those of children with NH. Evaluating the early communication skills of children with hearing loss in detail and forming children's therapy and education programs according to the results obtained will contribute to the language, speech and academic success of children in the future.
