ABSTRACT The overall prognosis of HCC is usually universally poor and hepatic resection with clear surgical margins is the only treatment modality that provides consistent, long-term, tumor-free survival. Unfortunately, only one quarter of patients with HCC may benefit from radical surgical treatments, largely because of the advanced stage of the disease and the accompanying cirrhosis at presentation. Chemoembolization has attracted much attention among several alternative treatments that have been assessed for more advanced HCC. Lipiodol is one of the most commonly employed materials for chemoembolization. Recent studies indicated that, HepG2 cells were capable of selective uptake of Lipiodol by endocytosis with prolonged intracellular retention, and cytotoxicity of Lipiodol was associated with lipid peroxidation in the cells. On the other hand, IFN-alfa was shown to inhibit growth of HCC cells by specifically delaying S- phase progression. Randomized controlled studies including Chinese patients reported positive survival benefit; however, the Western data derived from phase II investigations were not so encouraging because they described unacceptably high incidence of severe side effects that preclude treatment continuation. The current study was designed to investigate the time^and dose related cytotoxicity of IFN-alfa and Lipiodol, and delineated the potential additive effects associated with their combination. The combination models designed to settle the optimal sequence, in which, the HepG2 cells were incubated with IFN-alfa and Lipiodol concomitantly, or IFN-alfa was administrated following the incubation with Lipiodol did not alter the cytotoxicity. However, with the administration of Lipiodol following the incubation of HepG2 cells with IFN-alfa for 24 hours, we observed a significant additive effect in the cytotoxicity. Then the same sequential model was repeated using one third of the initial IFN-alfa dose, and an efficient cytotoxicity was achieved as well. We conclude that, a better understanding of the mechanisms by which IFN-alfa and Lipiodol interact will facilitate the development of effective strategies to lessen the side effects, and yield better therapeutic results allowing the maximum therapeutic dose of IFN-alfa to be administrated with increased availability.
