Madde kullanım bozukluğu olan bireylerin ailelerinin duygu ve davranışları ile baş etmelerinin nitel olarak araştırılması

Özet
Bu çalışma, madde kullanım bozukluğu olan bireylerin ailelerinin madde kullanımını ilk nasıl öğrendikleri, öğrendiklerinde ve süreç içerisindeki duygu ve davranış deneyimleri, bu problemin aile yaşamlarını nasıl etkilediği ve bu süreçte onlara iyi gelen şeylerin neler olduğunun incelenmesi amacıyla yapılmıştır. Araştırmanın örneklemini, gönüllülük esasına uygun olarak çalışmaya katılmayı kabul eden Ege Üniversitesi Tıp Fakültesi Ruh Sağlığı ve Hastalıkları Anabilim Dalına bağlı Madde Bağımlılığı Polikliniği ve Madde Bağımlılığı yataklı servis bölümünde tedavi görmekte olan, DSM-5 (Ruhsal Bozuklukların Tanısal ve Sayımsal El Kitabı - 5)’e göre Madde Kullanım Bozukluğu tanısı almış kişilerin aile bireyleri oluşturmuştur. Çalışmaya 9 gönüllü katılmıştır. Bu katılımcılar ile yarı yapılandırılmış görüşme formu kullanılarak derinlemesine görüşmeler yapılmıştır. Görüşmelerde katılımcıların izinleri alınarak ses kayıtları alınmış, ses kayıtlarının daha sonra transkripsiyonu oluşturulmuştur. Tüm görüşmeler içerik analiz tekniği ile incelenmiştir. Madde kullanım bozukluğu olan bireylerin ailelerinin bu süreçteki deneyimleri incelendiğinde, duygularının yoğun olarak üzüntü, hayal kırıklığı, çaresizlik, öfke olduğu ve davranışlarının ise araştırma yapmak, şiddet uygulamak, sakin kalmaya çalışmak, alttan almak olduğu belirtilmiştir. Ayrıca deneyimledikleri güçlüklerin bireysel, sosyal ve aile içi ve ekonomik güçlükler olduğu görülmüştür. Ek olarak madde kullanım bozukluğu olan bireylerin ailelerinin bu süreçteki duygu ve davranış deneyimleri ile baş etmek için ise yeterli baş etme yöntemlerine sahip olmadıkları görülmüştür. Baş vurulan yöntemler; eş, akraba veya arkadaş ile paylaşımda bulunmak, ruh sağlığı desteği almak ve spor yapmak olduğu belirtilmiştir. Dahası ailelerin genel olarak bu süreçte sadece madde kullanıcısının ruh sağlığının ve tedavisini önemsedikleri, kendilerine odaklanmadıkları ve duygularını çevreye belli etmemeye çalıştıkları görülmüştür. Ailelerin kendi duygu ve düşüncelerine odaklanmamasının madde kullanım bozukluğu olan bireyin tedavi sürecini olumsuz yönde etkileyeceği düşünülmektedir. Ailelerin sağlıklı baş etme stratejilerine sahip olmalarının, aile sistemini daha işlevsel hale getirebileceği ve kullanım bozukluğu olan bireyin tedavi sürecini olumlu yönde etkileyeceği düşünülmektedir. Bu çalışmanın madde bağımlılığı alan yazınına katkıda bulunduğu düşünülmektedir.
This study was conducted to analyzed how the families of individuals with substance use disorder found out about the related family member’s substance use, their emotional and behavioral experiences when the usage was known for the first time and during the process and how this problem affects the family’s life, and what is good for them in this process. The sample of the study is created by the volunteer families of individuals whose family members that have been diagnosed with Substance Use Disorder according to DSM-5 (The Diagnostic and Statistical Manual of Mental Disorders – 5) are being treated in the acute inpatient unit of Substance Addiction Polyclinic and Substance Addiction Acute Inpatient Unit in Department of Psychiatry, Medical Faculty of Ege University. Nine volunteers participated in the study. The interviews were conducted thoroughly with these participants using a semi-structured interview form. During the interviews, voice recordings were taken with the permission of the participants, and then the transcriptions of the voice recordings were formed. All interviews were analyzed by the technique of content analysis. When the experiences of the families of individuals with substance use disorder are investigated during the process, their emotions were mostly destructive emotions such as sadness, disappointment, helplessness, anger, and the first reaction of the families when they find about the usage are doing research, seeking treatment, resorting to violence, trying to stay calm. Also, the difficulties were experienced by the families are individual, economic, and social and domestic difficulties. In addition to these, it has been observed that the families of individuals with substance use disorder do not have enough coping strategies to deal with their emotions and behavioural experiences during the process. The methods that have been used are sharing with spouse, relatives or friends, getting mental health support and doing sportive activities. Moreover, it has been observed that families generally only care about the mental health and treatment of the substance user in this process, they do not focus on themselves and try to hide their feelings from their social the environment. It is thought that families not focusing their own feelings and thoughts shall affect the treatment process negatively. It is believed that families having healthy coping strategies can make the family system more functional and affect the treatment process of the individual with a usage disorder positively. This study is believed to contribute to the substance addiction literature.
