Objective: This study aimed to compare the early clinical and physiological efficacy of the portable Flow-Safe II+® BiPAP system and conventional non-invasive mechanical ventilators in patients presenting to the emergency department with acute cardiogenic pulmonary edema (ACPE) requiring non-invasive positive pressure ventilation (NPPV). Materials and Methods: In this prospective, observational comparative study, 180 patients (Flow-Safe II+®, n=90; conventional, n=90) who received NPPV for ACPE between August 1, 2024, and April 28, 2025, in the Emergency Department of Ege University Hospital were evaluated. Results: Vital signs, blood gas, laboratory values, and physical examination findings were recorded at baseline, 30, and 60 minutes. Mechanical ventilator duration, emergency department length of stay, and short-term outcomes (repeat NPPV, intubation, discharge, hospitalization, mortality) were analyzed. The two groups were largely homogeneous regarding age, gender, comorbidities, and initial vital signs. Baseline SpO₂ was lower in the Flow-Safe II+® group (median 84.5% vs 87.5%; p=0.018), while coronary artery disease (57.8% vs 38.9%; p=0.017) and BNP (median 5,886 vs 3,749 pg/mL; p=0.044) were higher in the conventional group. Flow-Safe II+® demonstrated significantly greater SpO₂ increases at 0–30 and 0–60 minutes (p=0.045 and p=0.037, respectively); no significant differences were found in blood gas or other vital/clinical variables and short-term outcomes. At 60 minutes, orthopnea and rhonchus were more frequent in the Flow-Safe II+® group (p=0.002 and p=0.009), but after multiple testing correction, only orthopnea remained significant. Conclusion: The Flow-Safe II+® portable BiPAP system was found to be as effective and safe as conventional non-invasive mechanical ventilators in patients with ACPE, offering an early advantage in oxygenation. Its portability, lack of need for electricity, and rapid setup make it a practical alternative in prehospital and emergency department settings with limited resources.
