Özet
AMAÇ: Gestasyonel diyabetes mellituslu gebelerde diyet ve egzersizin kan şekeri regülasyonundaki etkinliğini araştırmak. GEREÇ VE YÖNTEM: 24-32. gebelik haftaları arasında diyabetes mellitus tanısı konan 111 olgu çalışmaya alındı. Çalışma grubunu oluşturan 55 gebeye diyet ile birlikte egzersiz tedavisi başlandı, 56 olgu ise kontrol grubu olarak alındı ve yalnız diyet uygulandı. Tüm olguların tedaviye başlanmadan önce ve tedavinin 7.günü kan şekeri profilleri çıkartıldı. Gebeler doğuma kadar izlendi ve insülin tedavisi gereksinimi, doğum şekli, maternal ve fetal komplikasyonlar kaydedildi. Student-t testi ve chi-square testi istatistiksel değerlendirme için kullanıldı. BULGULAR: Yalnız diyet uygulanan 56 olgunun %32'sinde (18 olgu) kan şekeri regulasyonu sağlanamayıp insülin başlandı. Diyet ile birlikte egzersiz tedavisinin kullanıldığı 55 olgunun ise %9 'una (5 olgu) insülin tedavisi uygulandı (p=0.002). Birinci hafta sonunda, çalışma grubu ile kontrol grubu arasında açlık kan şekeri ortalamaları yönünden anlamlı fark saptanmadı (85±17 mg/dl, 88±32 mg/dl sırasıyla, p=0.16). Birinci hafta sonu 2.saat tokluk kan şekeri ortalamaları ise çalışma grubunda kontrol grubuna oranla belirgin olarak düşüktü (102±23 mg/dl 115±39 mg/dl, p<0.001). Çalışma grubunda sezaryen oranı % 43 olmasına karşın, kontrol grubunda % 70 düzeyindeydi (p<0.05). SONUÇ: Gestasyonel diyabetes mellituslu gebelerde diyet ile birlikte egzersiz kullanımı kan şekeri regulasyonu kolaylaştırmakta ve insülin kullanım oranını azaltmaktadır.
PURPOSE: To investigate the effect of diet and exercise on the regulation of blood glucose levels in patients with gestational diabetes mellitus. MATERIAL AND METHODS: 111 patients who were diagnosed as diabetes mellitus between 24th and 32nd weeks were included in this study. 55 patients in the study group were treated with diet and exercise together while 56 cases in the control group were treated with diet only. Daily blood glucose profiles of all cases were measured before therapy and at 7th day of the therapy. the pregnant women were followed until delivery and insulin needs, delivery type, maternal and fetal complications were noted. Student T test and chi-square tests were used for statistical analysis. RESULTS: Blood glucose regulation could not be achieved in 32% (18 cases) of 56 cases treated with only diet and insulin was started. But only 9% (5 cases) of 55 cases treated with diet and exercise together were performed insulin therapy (p=0.002). At the end of the first week, no statistically significant difference was found between the study and control groups in terms of mean fasting glucose levels. (85±17 mg/dl, 88±32 mg/dl respectively, p=0.16). At the end of the first week, glucose levels in the 2nd hour were significantly lower in the study group than the control group. (102±23 mg/dl, 115±39 mg/dl, p<0.001). Cesarean rate was 43% in the study group although it was 70 % in the control group. (p<0.05). CONCLUSION: the use of exercise therapy together with diet in pregnant woman with gestational diabetes mellitus makes the blood glucose regulation easier and decreases the need for insulin.
