Özet
Bazı parazit enfeksiyonları alerjik semptomlarla ortaya çıkmaktadır. Bu çalışmada alerjik yakınması olan hastaların gastrointestinal sistem ve iç organlarda yerleşen kist hidatik enfeksiyonu yönünden araştırılması amaçlanmıştır. Poliklinik laboratuvarımıza dermatoloji ve dahiliye polikliniklerinden alerji yakınması ile gelen dışkı incelemesinde parazit saptanan 22 ve parazit saptanmayan 23 olmak üzere toplanı 45 olgu çalışmaya dahil edilmiştir. Dışkı ve anal bant incelemesi sonucunda parazit saptanan hastaların 4'ünde Taenia saginata, birinde Hymenolepis nana, 9'unda Enterobius vermicıılaris, birinde Ascaris lumbricoides yumurtaları tespit edilmiştir. Ayrıca hastaların 3'ünde lodamoeba biitschlii, 3'ünde Entamoeba histolytica, 2'sinde Giardia intestinalis, l'inde de Blastocystis hominis kistleri saptanmıştır. İki hastada E. vertnicularis yumurtaları ve /. biitschlii kistleri ve bir hastada da 7". saginata yumurtaları ve E, histolytica kistleri birlikte görülmüştür. Tüm hastaların serumları, koyun kist sıvısı antijeni kullanılarak ELISA IgG ve indirekt hemaglütinasyon (IHA) yöntemleri ile Kist Hidatik antikorları yönünden incelenmiştir. ELISA yöntemi ile hastaların 3'ünde pozitif reaksiyon ve 4'ünde de zayıf pozitif reaksiyon tespit edilmiştir. IHA ile 7 hastanın sadece 4'ünde pozitiflik saptanmıştır. Hastaların periferik kan yaymaları incelendiğinde eozinofil değerleri, 10 hastada % 0-3, 12 hastada % 3-9 ve 9 hastada %9-25 arasında tespit edilmiştir. Alerjik yakınması olan hastalarda, paraziter enfeksiyonların ayırıcı tanıda göz önünde bulundurulmasının gerekli olduğu, bağırsak parazitleri yanında hastalarda iç organlara yerleşebilen ve alerjik yakınmalar oluşturabilen helmint enfeksiyonlarının da incelenmesinin yararlı olacağı sonucuna varılmıştır.
Some parasitic infections manifest allergic symptoms. The aim of this study was to investigate parasitic infections of the gastrointestinal system (GIS) and cystic echinococcosis (CE) of internal organs among patients with allergic symptoms. Forty-five patients from the Internal Medicine and Dermatology outpatient clinics presented to the coproparasitology laboratory with allergic complaints. Intestinal parasites were detected in 22 of them by stool and perianal material examination. Taenia saginata eggs were detected in 4 patients, Hymenotepis nana eggs in l, Enterobius vermicularis eggs in 9, and Ascaris lumbricoides eggs in 1. Also the cysts of lodamoeba biitschlii were detected in 3 patients, Entamoeba histolytica in 3, Giardia intestinalis in 2 and Blastocystis hominis in 1. Two out of 22 patients had 2 parasites in their stool and perianal material, 1 of them had E. vermieiilaris and 1. biitschlii cysts and the other had T. saginata eggs and E. histolytica cysts. CE antibodies were investigated in the sera of the patients using ELISA Ig G and IHA tests with sheep hydatid cyst fluid as an antigen. Positive reactions were found in 3 patients and a weakly positive reaction was found in 4 patients using the ELISA method. But positive reactions were found in only 4 of these 7 patients using the IHA test. The calculated eosinophil percentages in the peripheral blood smears of the patients ranged between 0-3% in 10 patients, 3-9% in 12 patients and 9-25% in 9 patients. These findings suggest that parasitic infections must be included in the differential diagnosis of the patients with allergic symptoms. Not only GIS parasites but also helminthic infections of the internal organs, which could cause allergic symptoms, should be investigated.
