Özet
Paranazal sinüslerin fungal enfeksiyonları sön yıllarda gittikçe artan sayıda bildirilmektedir. Fungal sinüzitlerin başlıca invaziv (akut, kronik) ve noninvaziv formları vardır. Noninvaziv fungal sinüzitler allerjik veya mycetoma şeklindedir. Mycetomalar tek sinüste yerleşerek, kronik sinüzit bulguları verirler ve büyük çoğunluğunda etken aspergillustur. Aralık 1993-Aralık 1996 yılları arasında Anabilim Dalımızda kronik sinüzit nedeniyle endoskopik sinüs cerrahisi uygulanan 204 olgunun dokuzunda mycetoma saptanmış ve hepsinde etken aspergillus olarak bulunmuştur. Mycetomların yerleşim yer¬leri; 4 olguda sfenoid sinüs, 2 olguda maksıller sinüs, 2 olguda etmoid sinüs, 1 olguda ise frontal sinüs olmuştur. Olguların hepsinde de sinüs mukozası histolojik olarak incelenmiş ve invazyon saptanmamıştır. Hastalarda fungal enfeksiyona neden olabilecek predispozan faktörler araştırılmış, immünolojik parametreler değerlendirilmiş ve normal bulunmuştur. Tüm olgulara fonksiyonel endoskopik sinüs cerrahisi uygulanarak, sinüs içindeki mycetomlar çıkartılıp, sinüs aerasyonu sağ¬lanmıştır. Ayrıca anti-fungal tedavi uygulanmamıştır. Hastaların kontrol BT'lerinde ve endoskopik nazal bakılarında rezi-div saptanmamıştır.
In recent years, fungal infections of the paranasal sinuses are being reported increasingly. Invasive (acute and chronic) and non-invasive fungal sinusitis are the two forms. Clinically, non-invasive sinusitis can be seen as allergic fungal sinusitis or fungus balls (mycetoma). Fungus balls are found in one sinus and represent the symptoms of chronic sinusitis. Mostly, the fungus balls are formed by aspergilius. Functional endoscopic sinus surgery (FESS) was performed in 204 patients between December 1993 and December 1996. Of these patients, we detected fungus balls in 9 cases. The cause was aspergillius in all cases. The localization of the disease was sphenoid sinus in 4 patients, maxillary sinus in 2 sinus, ethmoid sinus in 2 patients andfrontak sinus in 1 patient. Sinus mucosa was evaluated histopathologicaly and no tissue invasion was found. The immunologic parameters and predisposing factors were assessed in all patients. There was no abnormality. FESS was performed in all patients for the purpose of eradication of the fungus balls and ventilation of the sinuses. The pati¬ents did not receive additional antifungal medical therapy. In the follow-up period, no residive was detected in endoscopic controls and CT scans.
