Özet
Amaç: Pediatri hemşirelerinin iş yükü algısı ile standart önlemlere uyumları arasındaki ilişkinin belirlenmesidir. Yöntem: Çocuk birimlerinde çalışan pediatri hemşirelerinin iş yükünün standart önlemlere etkisinin değerlendirilmesi amacıyla yapılmış tanımlayıcı, prospektif, kesitsel bir araştırmadır. Çalışmanın verilerini toplamak için kullanılacak bilgilendirilmiş onam formu, katılımcı bilgi formu, standart önlemler ölçeği, iş yükü ölçeği kullanılmıştır. Google Formlar üzerinden oluşturulan anket formu, araştırmaya katılmayı kabul eden pediatri hemşirelerine mail ve whatsapp, facebook, instagram, telegram gibi sosyal medya üzerinden ulaştırılmıştır. Ardından bilgilendirilmiş onam formunu dolduran katılımcıların ankete devam etmesi sağlanmıştır. Yeterli örnekleme ulaşıldığında veri toplama aşaması sonlandırılmıştır. Bulgular: Araştırmaya katılan hemşirelerin iş yükü puan ortalaması 41,57±7,65 (min:15, max:55) olarak belirlendi ve hemşirelerin %32,7'si iş yükünü fazla, %48'i çok fazla bulduklarını belirtmiştir. Hemşirelerin iş yüklerini arttıran en önemli faktörün %82'si yetersiz hemşire sayısı, %77,7'si hemşirenin görev dışı işleri yapması olduğunu belirtmiştir. Erkeklerin iş yükü algıları kadınlardan yüksek bulunmuştur. Hemşirelerin çalıştıkları hastane türü ile iş yükü algısı arasında anlamlı fark bulunmuştur. Hemşirelerin aylık 3 ve daha fazla nöbet tutanlar ile alınan ücreti yeterli bulanların iş yükü algıları anlamlı olarak daha fazla olduğu bulunmuştur. Araştırmaya katılan pediatri hemşirelerinin standart önlemlere uyum puan ortalaması 13,45±3,82 olarak hesaplanmıştır. Hemşirelerin %89,3'ü izolasyonun hastane enfeksiyonunu önlemede gerekli olduğunu düşünmektedir. Katılımcıların %90,3'ü temas izolasyonu, %72,7'si solunum izolasyonu, %68,3'ü damlacık izolasyonunu uygulamışlardır. Pediatri hemşirelerinin izolasyon önlemlerini uygulama konusundaki sıkıntılarının başında hekimlerin izolasyon önlemlerine uyumsuzluğu ve malzeme eksikliği gelmektedir. Kadınların standart önlemlere uyumları erkeklerden anlamlı olarak daha fazla olduğu bulunmuştur. Standart önlemlere uyum ile yaş arasında anlamlı fark bulunmuştur. Hastane türü ile aylık tuttukları nöbet sayısı, standart önlemlere uyum arasında anlamlı fark bulunmamıştır. Çalışmaya katılan pediatri hemşirelerinin standart önlemlere uyumları ile işten memnuniyetleri arasında anlamlı fark bulunmuştur. İş yükü algısı ve standart önlemlere uyum arasında negatif ve çok zayıf düzeyde anlamlı olmayan bir ilişki bulunmaktadır. Sonuç: Pediatri hemşirelerinin iş yükü fazla veya çok fazla olduğu belirtilmiştir. Hemşirelerin, orta düzeyin üstünde iş yükü algısına ve standart önlemlere uyum düzeyine sahip oldukları bulunmuştur. Pediatri hemşirelerinde standart önlemlere uyum arttığında iş yükü algısı çok düşük bir ilişki ile azalmaktadır ancak bu azalma istatistiksel olarak anlamlı ölçüde değildir. Pediatri hemşirelerinin iş yükü algıları ile standart önlemler arasındaki ilişkisini inceleyen daha fazla sayıda çalışmalar ve randomize kontrollü deneysel çalışmaların yapılması önerilir. Objective: Determine the relationship is between the perception of workload of pediatric nurses and their compliance with standard precautions. Method: This is a descriptive, cross-sectional, prospective study conducted to examine the effect of the workload of pediatric nurses working in pediatric units on standard precautions. Informed consent form, participant information form, standard precautions scale, workload scale were used to collect the data of the study. The questionnaire form created on Google Forms was sent to the pediatric nurses who agreed to participate in the study via e-mail and social media such as whatsapp, facebook, instagram, telegram. Then, the participants who filled out the informed consent form were allowed to continue with the survey. The data collection phase was terminated when sufficient sampling was reached. Results: The mean workload score of the nurses participating in the research was determined as 41.57±7.65 (min:15, max:55) and 32.7% of the nurses had a high workload stated that too much and 48% too much. 82% of the most important factor increasing the workload of nurses stated that the number of nurses was insufficient, and 77.7% of them stated that the nurses did non-duty work. Workload perceptions of men were found to be higher than women. The difference between the nurses' perception of the hospital type and workload was found to be significant. The workload perceptions of the nurses who kept 3 or more shifts per month and those who found the wages sufficient were found to be significantly higher. The mean score of compliance with standard precautions of pediatric nurses participating in the study was calculated as 13.45±3.82. 89.3% of the nurses think that isolation is necessary to prevent nosocomial infection. 90.3% of the participants applied contact isolation, 72.7% applied respiratory isolation, 68.3% applied droplet isolation. The main problems of pediatric nurses in applying isolation precautions are physicians' non-compliance with isolation precautions and lack of materials. Compliance of women to standard precaution was found to be significantly higher than men. The difference between adherence to standard precautions and age significant. The difference between the type of hospital and the number of shifts they keep per month and compliance with standard precautions was not significant. The difference between the participants' compliance with the standard precaution and their job satisfaction found to be significant. There is a very weak negative and insignificant relationship between the perception of workload and compliance with standard precautions. Conclusion: It was stated that the workload of the pediatric nurses was too much or too much. Nurses were found to have a workload perception above the medium level and to have a level of compliance with standard precautions. When compliance with standard precautions increases in pediatric nurses, the perception of workload decreases with a very low relationship, but this decrease is not statistically significant. It is recommended to conduct more studies and randomized controlled experimental studies examining the relationship between pediatric nurses' perception of workload and standard precautions.
