Diyabet tedavisinde hasta uyumu ile tamamlayıcı ve alternatif tedaviye ilişkin tutumların incelenmesi

Özet
Amaç: Bu çalışma; diyabet tedavisinde tedaviye uyum ile tamamlayıcı ve alternatif tedaviye ilişkin tutumların belirlenmesi amacıyla tanımlayıcı bir çalışma olarak tasarlanmıştır. Yöntem: Araştırma 20 Mayıs - 20 Ağustos 2022 tarihleri arasında Ege Üniversitesi Hastanesi Endokrinoloji ve Metabolizma Diyabet Polikliniğinde yapılmıştır. Araştırmanın örneklemini araştırma tarihlerinde Endokrinoloji ve Metabolizma Diyabet Polikliniğine başvuran, araştırmaya dâhil edilme kriterlerine uyan 150 hasta oluşturmuştur. Veriler anket yöntemi ile toplanmıştır. Araştırma verilerinin toplanmasında, "Birey Tanıtım Formu", "Bütüncül Tamamlayıcı ve Alternatif Tıbba Karşı Tutum Ölçeği (BTATÖ)" ve "Tip 2 Diyabet Tedavisine Hasta Uyum Ölçeği (Tip2DMHUÖ)" kullanılmıştır. Araştırma için bilimsel etik kuruldan, araştırmanın yapıldığı kurumdan ve araştırma kapsamına alınan hastalardan izin alınmıştır. Verilerin değerlendirilmesinde; Veriler SPSS 22 programından yararlanılarak sayı, yüzdelik dağılım, Mann Whitney U, Kruskal Wallis, Student t Testi ve Spearman korelasyon testi ile analiz edildi. Bulgular: Hastaların %77,3'ünün 51-60 yaş arasında olduğu, %70'nin kadın, %87,3'ünün evli, %41,3'ünün lise mezunu, % 56'sının emekli, % 46,7'sinin orta düzeyde gelire sahip olduğu ve %40,7'sinin ilçede yaşadığı belirlendi. Hastaların hastalık özelliklerine ilişkin olarak; %66'sının diyabete ek olarak kronik rahatsızlığının olduğu ve kronik hastalığı olanlar arasında en yüksek oranla (%31,3) hipertansiyon tanısının yer aldığı saptandı. Hastaların %44'ünün diyabet tedavisi için oral tablet aldığı, %60,7'sinin diyabet hakkında bilgi aldığı, % 51,7'sinin aldıkları bu bilgiyi yeterli bulduğu, %52'sinin kan şekerini düzenli ölçtüğü bulundu. Hastaların BTATÖ puan ortalaması toplam ölçekte 34,01±10,91; "Tamamlayıcı ve Alternatif Tıp" boyutunda 18,74± 8,69; "Bütüncül Sağlık" boyutunda 15,27± 6,95 olarak saptandı. Hastaların tanımlayıcı özelliklerinden cinsiyet, medeni durum, ekonomik durum, yaşadığı yer ve meslek durumunun BTATÖ alt boyutları ve toplam puan ortalamalarını etkilemediği saptandı (p>0,05). Hastaların yaş, eğitim ve bilgi alma durumunun; tamamlayıcı ve alternatif tıp alt boyut ve BTATÖ Toplam puan ortalamalarını etkilediği saptandı (p<0.05). Tip2DMHUÖ puan ortalamaları toplam ölçekte 86,74 ± 13,10; "Tutum ve Duygusal Etmenler" boyutunda 20,44 ± 6,48; "Yaşam Tarzı Değişiklikler" boyutunda 7,97 ± 3,50; "Öfke Duyguları" boyutunda 9,22 ± 3,60; "Uyuma Uygun Duygu ve Davranışlar "boyutunda 12,89 ± 4,72; "Diyet Pazarlığı" boyutunda 8,67 ± 1,57; "İnkâr Duygusu" boyutunda 9,95 ± 1,47 olarak bulundu. Hastaların cinsiyetinin, yaşının, medeni ve ekonomik durumunun, yaşadığı yer ve mesleğinin Tip2DMHUÖ toplam ve alt boyut puanını etkilemediği saptandı (p>0,05). Hastaların eğitim durumunun ise sadece "bilgi ve kişisel etmenler" alt boyut puan ortalamalarını etkilediği saptandı (p<0.05). Hastaların diyabet süresi ile ek kronik hastalığı olma, durumunun Tip2DMHUÖ alt boyut ve toplam puan ortalamalarını etkilemediği saptandı (p>0,05). Hastaların tedavi biçiminin uyum ölçeği toplam puan ortalamalarını etkilediği saptanırken, hastalık süresinin bilgi ve kişisel etmenler alt boyut puanını etkilediği saptanmıştır (p<0,05). Hastaların diyabetle ilgili bilgi alma ve kan şekerini düzenli ölçme durumlarına göre "bilgi ve kişisel etmenler", "uyuma uygun duygu ve davranışlar" ve "inkâr duygusu" alt boyut puan ortalamaları arasında anlamlı farklılık görülmüştür (p<0,05). Hastaların BTATÖ toplam puanı ile Tip2DMHUÖ toplam puan ortalamaları arasında ilişki incelenmiş olup, ölçek toplam puan ortalamaları arasında anlamlı bir ilişki bulunmamıştır (p>0,05). Sonuç: Bu çalışma sonucunda, hastaların BTATÖ toplam puana göre tutumun pozitif ve orta düzeyin üzerinde olduğu, Tip 2 Diyabet Tedavisine uyumun orta düzeyde olduğu; eğitim, hastalara uygulanan tedavi yöntemleri, hastaların tedavi süresi, diyabetle ilgili bilgi alma durumu ve kan şekerini düzenli ölçme değişkenlerinin puanları etkilediği görülmüştür. Objective: This study was designed as a descriptive study in order to determine attitudes towards treatment compliance and complementary and alternative treatment in diabetes patients. Methods: The research was conducted at the Endocrinology and Metabolism Diabetes Outpatient Clinic of Ege University Hospital where the study was conducted between 20 May -20 August 2022. The sample of the study consisted of 150 patients who applied to the Endocrinology and Metabolism Diabetes Outpatient Clinic and met the criteria for inclusion in the study. The data were collected by questionnaire method. "Individual Introduction Form", scale of attitude toward Holistic Complementary and Alternative Medicine Questionnaire (HCAMQ) and "Patient Adaptation to Type 2 Diabetes Treatment Scale (T2DTPAS)" were used in the collection of research data. Permission was obtained from the scientific ethics committee, the institution where the research was conducted and the patients included in the research for the research. In the evaluation of the data; The data were analyzed by using the SPSS 22 program with number, percentage distribution, Mann Whitney U, Kruskal Wallis, Student t Test and Spearman correlation test. Results: It was determined that 77.3% of the patients were between the ages of 51-60, 70% were female, 87.3% were married, 41.3% graduated from high school, 56% retired, 46.7% had a Decadal income and 40.7% lived in the district. Regarding Decease characteristics of patients; It was found that 66% of them had chronic disease October in addition to diabetes and hypertension diagnosis was included with the highest rate (31.3%) among those with chronic disease. It was found that 44% of patients took oral tablets to treat diabetes, 60.7% received information about diabetes, 31% found this information sufficient, and 52% regularly measured blood sugar. The mean HCAMQ score of the patients was found to be 34.01±10.91 on the total scale; 18.74± 8.69 on the "Complementary and Alternative Medicine" dimension; 15.27± 6.95 on the "Holistic Health" dimension. It was found that gender, marital status, economic status, place of residence and occupational status did not affect the HCAMQ sub-dimensions and total score averages of the descriptive characteristics of the patients (p>0.05). It was found that the age and educational status of the patients affected the mean scores of the complementary and alternative medicine sub-dimensions and HCAMQ Total (p<0.05). Total scores on the scaleT2DTPAS 86,74 ± 13,10; "attitude and emotional factors" size 20,44 ± 6,48; "lifestyle changes" size 7,97 ± 3,50; "feelings of anger," size 9,22 ± 3,60; "appropriate feelings and behaviors of sleep "size 12,89 ± 4,72; "a bargain diet" size 8,67 ± 1,57; "denial" size 9,95 ± 1,47 was found. It was found that the gender, age, marital and economic status, place of residence and profession of the patients did not affect the total and sub-dimension scores of the T2DTPAS (p>0.05). It was found that the educational status of the patients affected only the mean scores of the "information and personal factors" sub-dimension (p<0.05). It was found that the duration of diabetes and the condition of having an additional chronic disease did not affect the T2DTPAS sub-dimension and total score averages (p>0.05). While it was found that the type of treatment of the patients affected the total score averages of the compliance scale, it was found that the duration of the illness affected the information and personal factors sub-dimension score (p<0.05). According to the patient's information about diabetes and regular measurement of blood sugar, there was a significant difference between the mean scores of the "information and personal factors", "feelings and behaviors suitable for Decency" and "sense of denial" sub-dimensions (p<0.05). The relationship between the total BTATÖ score of the patients and the total mean scores of the Type 2 DTPAS was examined, and there was no significant relationship between the total mean scores of the scale (p>0.05). Conclusion: As a result of this study, it was found that the attitude of patients according to the total HCAMQ score was positive and above the moderate level, compliance with Type 2 Diabetes Treatment was moderate; education, treatment methods applied to patients, duration of treatment of patients, the state of information about diabetes and regular measurement of blood sugar variables affected the scores.
