    Diagnostic accuracy of the mean platelet volume in the prediction of upper urinary tract infections

    Background: The studies have reported that mean platelet volume may be a marker in the discrimination of upper and lower urinary tract infections. We investigated whether the mean platelet volume was a reliable indicator in upper and lower urinary tract infections. Methods: One hundred and eighty-two patients between the ages of 1 month and 14 years who were diagnosed with urinary tract infection were included in this study. The mean platelet volume values, clinical and other laboratory characteristics of the groups were compared. The discriminative ability of each biomarker for patients with UUTI was evaluated by drawing receiver operating characteristic (ROC) curves for the biomarkers. All values of p < 0.05 were considered statistically significant. Results: The leukocyte count, C-reactive protein, and erythrocyte sedimentation rates of the patients with upper urinary tract infection were significantly higher than the patients with lower urinary tract infections (p < 0.001). However, there was no statistically significant difference in terms of the mean platelet volume values between patients with upper and lower urinary tract infection ([7.2 (1.3) vs. 7.2 (1.6) fL]; p = 0.79, respectively). The value of the area under the curve of MPV's p-value was > 0.05. However, p-values of CRP and ESR were significant in ROC analysis. Conclusions: Our study showed that there was no significant difference in the mean platelet volume values between patients with upper and lower urinary tract infections. The use of mean platelet volume as an indicator of upper urinary tract infection is controversial. © 2020 Verlag Klinisches Labor GmbH. All rights reserved.

    Duane retraction syndrome in a patient with abnormal head position [Anormal baş pozisyonu ile gelen hastada Duane retraksiyon sendromu]

    Duane’s syndrome is a rare retraction anomaly characterized by an innervation defect in the lateral rectus muscle, limitation of abduction and adduction due to the result of abnormal innervation of the horizontal rectus muscles, changes in the eyelid fissures, and abnormal vertical eye movements. The affected eye is displaced up and/ or down in adduction. This syndrome, also known as Stilling-TurkDuane syndrome, accounts for approximately 1 to 5% of all strabismus cases. In this article, we present a one-and-a-half-year-old male patient who had abnormal head position, and was diagnosed as having Duane retraction syndrome.     Outbreak of varicella in preschool children despite one-dose vaccination

    In Turkey, a single-dose varicella vaccine was introduced into the National Immunization Program in 2013. Before this implementation, varicella vaccine had been available in the private sector since 2000. However, varicella outbreaks continued to occur in preschools and elementary schools. We investigated a varicella outbreak to estimate the effectiveness of 1-dose varicella vaccine and to evaluate potential risk factors for breakthrough disease. This study was carried out during a varicella outbreak in 3 preschools in İzmir, Turkey, in April 2016. Using questionnaires, data including children’s medical and vaccination histories were collected from their parents. Attack rates in vaccinated and unvaccinated children were calculated and the analyses of vaccine effectiveness and of risk factors for breakthrough disease were conducted. A total of 124 children were enrolled in the study. Of the 124 children, 77 (62%) had received 1-dose varicella vaccine before the outbreak. Varicella developed in 34 of 124 children during the outbreak, and 18 of them (53%) had breakthrough varicella. The attack rate was 23.4% among vaccinated children and 34% among unvaccinated children. The effectiveness of single-dose varicella vaccine was 33.6% against varicella disease of any severity and 82.5% against moderate or severe varicella. Children vaccinated 5 or more years before the outbreak had 3.5 times the risk of disease than those who had been vaccinated more recently (OR 3.5 [95% CI, 1.08-11.5]); p= 0.046). Age at vaccination (<15 months vs.?15 months) and the brands of varicella vaccine were not associated with the increased risk of breakthrough varicella. Our study suggests that one-dose of varicella vaccine is not sufficient to prevent school outbreaks. A 2-dose varicella vaccination program may help to prevent varicella outbreaks and achieve effective control of the disease. © 2018, Turkish Journal of Pediatrics. All rights reserved.

    A rare cause of lymphadenopathy: Dermatopathic lymphadenitis: Case report [Nadir Bir Lenfadenopati Nedeni: Dermatopatik Lenfadenit]

    Dermatopathic lymphadenitis (DP), which is a lymphatic dishyperplasia, is usually seen after eczematoid or erythrodermic skin lessions. It is rare in childhood. Dermatopathic lymphadenitis can be seen after Epstein-Barr virus (EBV), human herpes virus 6 and 7 (HHV-6, HHV-7) infections, human papillomavirus (HPV) infections, also may occur for any reason without inflammation of the skin. Rarely it can be observed without signs of skin. The diagnosis of dermatopathic lymphadenitis is based on histopathological investigation. It is characterized by hyperplasia and infiltration of the follicular dendritic cells and internal paracortical distance of the lymph nodes. There is no specific treatment, it can regress spontaneously. In this article, we presented a 14 months-old case who had been admitted to our clinic with rash, fever and axillary lymphadenopathy, and was diagnosed with dermatopathic lymphadenitis. © Copyright 2016 by Türkiye Klinikleri.

    Use of Dietary Supplements and Influencing Factors in Children

    Introduction: In recent years, the use of dietary supplements has increased in all age groups. Parents may also use these supplements for their children for different reasons. This study aims to determine the use of dietary supplements by children, the factors affecting this use, and the attitudes of parents about these products. Methods: A total of 1038 children aged 2–18 years without any chronic disease who presented to the pediatric outpatient clinics of Ege University Children’s Hospital were included in this study. Parents (n = 1000) who agreed to participate in the study were interviewed face-to-face, and a comprehensive questionnaire including questions about children’s use of dietary supplements, sociodemographic characteristics, and parents’ attitudes towards dietary supplements was administered. Analyses were performed with SPSS 25.0. Results: The mean age of the children included in our study was 8.6 ± 4.8 years, and 51% (n = 510) were male. It was found that 32.5% of the children used nutritional supplements, and vitamin–mineral preparations (23.2%) were the most frequently used. Omega-3 (19.3%) and immune support products (9.4%) were the second and third most frequently used supplements, respectively. A significant relationship was found between the use of dietary supplements and the child’s age, body weight, body mass index, parents’ educational level, being health worker, and economic status (p < 0.05). It was found that most of the families thought that vitamin–mineral and omega-3 products were beneficial for growth and development and that they received information from doctors most frequently before taking these products. However, it was found that families followed the media as the second most frequent source of information for these products. Conclusions: Approximately one-third of the children in our study use dietary supplements. It is very important to raise awareness among families about the use of these products when necessary and with the recommendation of a physician. To prevent families from using dietary supplements that are not necessary for their children, especially due to misinformation in the media, pediatricians should provide correct information to parents about these products at every clinic visit. A concerted effort is needed from policy makers, media organizations, and health care providers to guide the safe use of DS. The results obtained from this study will shed light on future randomized controlled prospective studies. © 2024 by the authors.

    A very rare cause of childhood paraparesis: Primary intradural extramedullary spinal hydatid CYST

    Hydatid disease is caused by the larval form of Echinococcus. Lung and liver are the most commonly affected sites. Primary intradural extramedullary hydatid disease is extremely rare; a 13-year-old girl with primary intradural hydatid cyst who presented with symptoms of paraparesis is discussed in this article.
