Öğe Antenatal Findings of Keratitis-Ichthyosis-Deafness Syndrome(Elsevier Inc, 2020) Okmen F.; Hortu I.; Jafarova U.; Imamoglu M.; Ekici H.; Ergenoglu A.M.Background: Keratitis-ichthyosis-deafness (KID) syndrome is a congenital ectodermal disorder characterized by keratitis, ichthyosis, and deafness. This syndrome affects multiple systems and can be fatal. Case: A 34-year-old G2, P1 woman was admitted to the Ege University School of Medicine in Izmir, Turkey because of a rapid increase in abdominal circumference at 32 weeks gestation. Fetal anatomic screening revealed complete chorioamniotic separation, hypoplasia of the cerebellar vermis, and dysmorphic facial findings such as frontal bulging. After the delivery, the baby's whole body had granular thickened skin. Bilateral dry eye, corneal edema, and bilateral retinopathy of prematurity were diagnosed. Conclusion: This case report highlights the importance of prenatal diagnosis through ultrasonography and magnetic resonance imaging. Karger AG, 2017) Taylan E.; Akdemir A.; Ergenoglu A.M.; Yeniel A.O.; Tekindal M.A.Aims: The study aimed to investigate whether we can predict the presence and severity of intra-abdominal adhesions before cesarean delivery using patient history, symptoms, and abdominal skin scar characteristics. Methods: In this prospective study, 143 pregnant women with history of previous abdominal surgery were included and they delivered by cesarean. Preoperative abdominal scar characteristics and symptoms as well as intraoperative abdominal adhesions were evaluated using the Manchester Scar Scale, a symptomatology questionnaire and the More Comprehensive Adhesion Scoring Method, respectively. Results: Patients with adhesions (n = 98) and without adhesions (n = 45) had similar baseline characteristics. In the adhesion group, abdominal scar scoring parameters were significantly increased. However, there was no significant correlation among total scar score, adhesion score, and symptom score. Conclusion: Despite the availability of many proposed methods, accurate prediction of the severity of surgery-related adhesions is beyond our current abilities. Therefore, as healthcare providers, obstetricians should avoid unnecessary use of the cesarean approach. This approach is more effective, beneficial, realizable, and reasonable than the prediction of surgery-related adhesions. © 2016 S. Prior surgery specially tubal surgery is one of the most important risk factors. We presented heterotopic pregnancy with literature that was in reproductive age and had prior ectopic pregnancy history treated by salpingostomy. In this patient had a succesful vaginal delivey before ectopic pregnancy. Choice of lineer salpingostomy in treatment of ectopic pregnancy should be discussed with patient and her husband for future fertility. Next pregnancy of in these patient should be ectopic pregnancy and heterotopic pregnancy that we presented.Öğe Comparison of the efficacy of seprafilm, interceed, adept, preclude membrane and spraygel in a rat model(2012) Sanhal C.Y.; Cirpan T.; Ergenoglu A.M.; Yeniel A.O.; Akdemir A.Objective. To compare the efficacy of five adhesions barrier products with a control group in a rat model. Methods. In this randomized controlled animal study, standardized lesions on parietal peritoneum of rats were created by sandpaper and three interrupted sutures. Afterwards, the lesions were treated with Seprafilm, Interceed, Preclude membrane, Adept, SprayGel or no barrier. The resulting adhesions were examined 21 days postoperatively. Adhesion quantity was assessed using a fully quantitative score calculated as the summated width of individual adhesions divided by the width of the traumatized area. Results. Compared to the control group, only SprayGel revealed a statistically significant reduction in adhesion quantity. There were also no statistically significant differences between groups. Conclusion. The promising efficacy of SprayGel (95% decrease in adhesion quantity) warrants the further evaluation of this product in larger human trials.Öğe Comparison of three-dimensional ultrasound and magnetic resonance imaging diagnosis in surgically proven Müllerian duct anomaly cases(Elsevier Ireland Ltd, 2016) Ergenoglu A.M.; Sahin Ç.; Şimşek D.; Akdemir A.; Yeniel A.Ö.; Yerli H.; Sendag F.Objective To determine and compare the diagnostic accuracy of 3-dimensional ultrasound (3D US) and magnetic resonance imagining (MRI) in patients with surgically diagnosed Mullerian duct anomaly (MDA). Study design Charts of patients with MDA were retrospectively evaluated. Patients who underwent both laparoscopic and hysteroscopic surgery and had 3D US and MRI examinations were included in the study. The diagnoses achieved via 3D US and MRI were compared with the surgical diagnoses to determine the diagnostic accuracy of these imagining techniques. Results Twenty-nine patients were included in the study. Three-dimensional ultrasound detected 28 out of 29 (96%) patients correctly. Only one patient was diagnosed with a uterine septum instead of uterine arcuatus. Magnetic resonance imaging detected 23 out of 29 patients correctly (79%). The Kappa indexes of the 3D US and MRI were 0.896 and 0.592, respectively. Conclusion Our results indicate that 3D US has a higher diagnostic accuracy level than MRI in evaluating MDA, especially when used in experienced hands. However, additional, well-designed studies are needed to better compare the diagnostic accuracy of the 3D US and MRI. © 2015 Published by Elsevier Ireland Ltd. Because, clinic examinations, laboratory findings are different and all of the radiologic imaging techniques can not be used in pregnancy. In order to this, complications of appendicitis may occur. One of the most important complications of the appendicitis is perforated appendicitis. We reported two pregnant women who operated in the third trimester for perforated appendicitis and had diffuse peritonitis and peritoneal abscess after surgery. In pregnancy, preterm birth rate is higher after perforated appendicitis. Difficulties in diagnosis make appendicitis more complicated with pregnancy and fetal outcome become worse.Öğe Diagnosis of bladder carcinoma in a woman presenting with postmenopausal bleeding: Case report(Turkiye Klinikleri, 2015) AkdemIr A.; Zeybek B.; Taylan E.; Ergenoglu A.M.; YenIel A.Ö.Although the most common etiology for postmenopausal bleeding is from genital organs, rarely various lesions from adjacent tissues such as urinary tract or distal gastrointestinal tract may cause similar symptomatology that mimics postmenopausal bleeding. We report a case of patient, which was initially evaluated as postmenopausal bleeding through consecutive diagnostic procedures, and finally hysteroscopy and co-existent cystoscopy that revealed in situ bladder carcinoma. When evaluating postmenopausal bleeding especially in elderly women, bladder tumor should be kept in mind while establishing differential diagnosis, and in addition to transvaginal ultrasonography and hysteroscopy, cystoscopy should be performed for further investigation particularly in patients with recurrent postmenopausal bleeding in the absence of gynecological pathology. ©2015 by Türkiye Klinikleri.Öğe Does high maternal first trimester iron status have an effect on the 50 g oral glucose test?(2012) Yeniel A.Ö.; Ergenoglu A.M.; Sanhal C.Y.; Sahin C.; Ulukus M.; Oztekin K.We aimed to test the hypothesis that 1st trimester high body iron status is associated with a high positive 50 g oral glucose tolerance test. In this study, 29 pregnant women with positive 50 g oral glucose tolerance test were compared with 94 negative 50 g OGTT patients as the control group in terms of 1st trimester iron status. Both groups had similar age, weight, height, body mass index and also median gravidity and parity values. Our results showed that there were no differences between groups in mean haemoglobin, haematocrit, serum iron, serum ferritin, total iron binding capacity and transferrin. Since it seems that free radicals have much influence on oxidative stress and glucose metabolism, prospective, randomised clinical trials should be designed to demonstrate the possible relation between maternal iron status and glucose intolerance. © 2012 Informa UK, Ltd.Öğe Early diagnosis of rudimentary horn pregnancy and its excision by laparoscopy [Rudimenter horn gebeliginin erken tanisi ve rudimenter hornun laparoskopik eksizyonu: Olgu Sunumu](2012) Ergenoglu A.M.; Yeniel A.O.; Yildirim N.; Sagol S.; Itil I.M.Rudimentary horn is one of the mullerian canal anomalies, the probability of rudimentary horn pregnancy is between 1/76000 and 1/150000. Usually, it is diagnosed after the rupture. In this case report, a patient who had a cesarean section before and who had an early diagnosis of rudimentary horn pregnancy in the eighth pregnancy week is mentioned. The rudimentary horn is excised by laparoscopy in this patient.Öğe Effect of oxytocin treatment on explant size, plasma and peritoneal levels of MCP-1, VEGF, TNF-? and histopathological parameters in a rat endometriosis model(Elsevier Ireland Ltd, 2014) Yeniel A.Ö.; Erbas O.; Ergenoglu A.M.; Aktug H.; Taskiran D.; Yildirim N.; Ulukus M.Objective: To determine the effects of oxytocin (OT) on surgically induced endometriosis in a rat model. Study design: Twelve female Sprague-Dawley rats were included. After the implantation and establishment of autologous endometrium onto the abdominal wall peritoneum, the rats were randomly divided into two groups, treated with intramuscular oxytocin (OT group, 160 µg kg/day, n = 6) or isotonic NaCl solution (control group, 1 mL kg/day, n = 6) for 28 days. To evaluate the therapeutic effects of OT, the explant volumes were calculated and the levels of vascular endothelial growth factor (VEGF), monocyte chemotactic protein-1, and TNF-? were measured in plasma and peritoneal fluid. Endometriotic explants were examined histologically by semiquantitative analysis. Results: After treatment, the mean endometriotic explant volume was decreased in the OT group (p = 0.016). The histopathological score and VEGF immunoexpression of endometriotic explants were significantly lower in the OT group (p = 0.007) than in controls (p = 0.000). Inflammatory cytokine levels in plasma and peritoneal fluid were considerably decreased in the OT group. Moreover, TUNEL immunohistochemistry clearly demonstrated more apoptotic changes in the mononuclear cells of the OT group compared with controls. Conclusion: We suggest that oxytocin might be considered as a potential candidate therapeutic agent for endometriosis. © 2014 Elsevier Ireland Ltd. All rights reserved.Öğe Effect of placenta previa on fetal growth restriction and stillbirth(2012) Yeniel A.O.; Ergenoglu A.M.; Itil I.M.; Askar N.; Meseri R.Aim: To examine the association between placenta previa and adverse perinatal outcomes such as low birth weight, preterm delivery, stillbirth and fetal growth restriction (FGR). Methods: This retrospective cohort study includes 12,034 delivered pregnant women who were recruited for the study between 2004 and 2010 in Ege University Hospital. Data were collected by browsing the clinic's archives. The association between placenta previa and adverse perinatal outcomes was determined via Chi-square tests and Student's t test. Logistic regression analysis was used to adjust for confounding factors in evaluating the association between placenta previa and the adverse perinatal outcomes. Results: There was no significant relationship between placenta previa and FGR or stillbirth. Low birth weight and preterm delivery were significantly higher in the placenta previa group. According to logistic regression analysis, low birth weight was associated with an OR of 3.01 (95 % CI 2.05-4.52) and preterm delivery was associated with an OR of 8.14 (95 % CI 5.60-11.83); while, placenta previa did not affect FGR and stillbirth significantly. Conclusion: Although there is no consensus on the association between placenta previa and FGR in previous studies, we suggest that placenta previa is not a reason for placental insufficiency. Management of placenta previa especially depends on maternal hemodynamic parameters such as heavy hemorrhage and hypotensive shock rather than fetal well-being protocols based on serial growth ultrasound and fetal Doppler investigation. © Springer-Verlag 2012.Öğe Effects of liver diseases on pregnancy and perinatal results: Comparison of 161 pregnant women with liver disease to 180 healthy pregnant [Karaci·ger hastaliklarinin gebeli·k ve peri·natal sonuçlar üzeri·ne etki·si·: 161 Karaci·ger hastalikli gebeni·n 180 saglikli gebe i·le karşilaş tirilmasi](2010) Ergenoglu A.M.; Yeniel A.Ö.; Sanhal C.Y.; Kazandi M.Objective: To determine the demographic properties, clinical features, laboratory findings and perinatal results of the patients who had liver diseases during their pregnancies. Material and methods: We retrospectively evaluated the medical records of 12241 pregnant women who had been treated in our clinic between January 2004 and July 2009. 161 of them (study group) had liver disease. For the control group, 180 patients, who did not have any problems on their routine controls, were chosen. Age, gravida, parity, gestational week, amnion fluid levels, birth week-weight-route, APGAR values of the first and fifth minutes, ALT, AST, ALP, GGT, total protein, albumin, globulin, total-direct-indirect bilirubin, INR, APTT, leucocyte, heamoglobuline, heamotocryte, platelets, blood urea, creatinine and sodium values were examined. Results: Demographic and clinical variables did not differ between groups. There was statistically significant difference in ALT, AST, ALP, GGT, total protein, albumin, total-direct-indirect billurubin, leucocyte, INR and creatinine values. We divided the study group in to 4 sub-groups; hepatitis B porters, cholestasis, asymptomatic liver function test abnormalities and others. We did not find any differences between groups in all parameters. Conclusion: The obstetrical complications of liver diseases in pregnancies may sometimes cause severe damages. With appropriate management and treatmant modalities, satisfying perinatal results could be gained.Öğe Granuler cell tumor of vulva: A case report and review of the literature [Vulvanin granüler hücreli tümörü: Olgu sunumu ve literatürün gözden geçirilmesi](Gunes Kitap Kirtasiye, 2014) Hortu I.; Ergenoglu A.M.; Zeybek B.; Akdemir A.; Yeniel A.Ö.Granular cell tumors are unusual neoplasms presumably derived from Schwann cells. These tumors are mostly seen in head and neck area and 5.3%of cases are localized in the vulva. Although most of the cases are benign, macroscopically they are poorly circumscribed and show adherence to the surrounding connective tissue. Malignant transformation was also reported and recurrence is common in these patients. Imperforate hymen is reported %0,1 of all newborns and it is think that cause of imperforate hymen is congenital defect in the degeneration of central cells of hymen. Although it is generally isolated anomally, sometimes genitourinary anomalies may associate this defect. Patients who has this anomally are generally asymptomatic until puberty and in the period of adolescence, patients administer with a primary amenore and cyclic pelvic pain. History and pelvic examination are the main steps in diagnosis. To determine possible associated anomalies imaging studies are usefull. Treatment of this anomally consist of vaginal reconstrictive surgery. We presented three imperforate hymen cases with literature in this case report.Öğe Isolated torsion of the fallopian tube with ectopic pregnancy: Case report(Turkiye Klinikleri, 2014) Yildirim N.; Yildirim A.G.S.; Akdemir A.; Ergenoglu A.M.; Yeniel A.O.Isolated tubal torsion containing an ectopic pregnancy is a very rare event. Ectopic pregnancy may also cause these symptoms and may impede the diagnosis of tubal torsion when these two coexist. We present a case with isolated fallopian tube torsion containing an ectopic pregnancy. A 25-year-old gravida 2, parity 0 woman presented with left lower abdominal pain and vaginal bleeding. Sonography revealed a 2 cm adnexal mass adjacent to the left ovary. ß-hCG level was 157 mIU/ml. Laparoscopy depicted torsion of the left fallopian tube together with an ectopic pregnancy. Left salpingectomy was performed. The clinician should also think about tubal torsion when the pain is intermittent and if he suspicious about the ectopic pregnancy. Material and methods: 65 women with placenta previa scheduled for elective or emergency cesarean sections from 2004 to 2009 were examined. Patient demographic data, surgery and obstetric characteristics, anesthetic techniques, blood transfusions, the values of hemoglobin and complications were recorded. Results: Mostly, general anesthesia was preferred in the parturients with placenta previa (86.2%, 56/65). 9 patients (13.8%), 2 of whom were converted to general anesthesia due to bleeding and prolonged surgery, received regional anesthesia. 37 of 65 women (56.9%) with placenta previa had had cesarean sections previously. More than half of these patients (21/37, 56.7%) had abnormally invasive placentation and 16 of 21 cases underwent cesarean hysterectomy. The incidence of complications in women with previous cesarean section with abnormally invasive placentation was higher than in the other women (p<0.0001). Three patients were transferred to the intensive care unit; 2 were intubated and mechanical ventilation was applied, and 1 died of hemorrhage. Conclusions: Anesthetic management is important for parturients with placenta previa who had previous cesarean section or abnormally invasive placentation. We found that general anesthesia was our method of preference for placenta previa as we wished to avoid the risk of bleeding. It occurs 12-13 days after fertilization via incomplete and late division of monozygotic embryonic disc. They can be named with depending on the anatomical location of the sticking points of twin pairs. Furthermore it may accompany other malformations. Parasitic twinning (heteropagus twinning) is a much rarer form of conjoined twinning. In this case report, a patient who was diagnosed with neural tube defect and rachipagus twinning at 19 weeks presented. Copyright © 2016 by Türkiye Klinikleri.Öğe Ovarian fibroma with metaplastic bone tissue: Case report(2011) Ergenoglu A.M.; Yeniel A.Ö.; Zekioglu O.Fibromas are benign tumors of the ovary and arising from the stromal component of the ovary. Nearly half of the cases (40%) have tumor bigger than 10 cm in diameter. Only one third of the cases have smaller than 3 cm and they are rarely bilateral (5% of cases) In microscopic examination fibromas consist of cellular bundles and intersecting strips of hyaline-appearing collagen and fibrous tissue. Mitoses are rare, but occasionally fibromas may be hypercellular and/or show substantial mitotic activity. Dystrophic calcifications, focal necrosis and hemorrhage are common. In this report we described a case of unilateral fibroma of ovary that was found incidentally by laparoscopic surgery for chronic pelvic pain with pathologically consisting metaplastic bone tissue.
