    Application of endoscopic hemoclips for nonvariceal upper gastrointestinal bleeding in children

    Background/Aims: Data about the efficiency and outcome of therapeutic endoscopic techniques in children with nonvariceal upper gastrointestinal bleeding (UGB) are scarce. We aimed to analyze our experience with endoscopic hemoclip application in children with non-variceal UGB.Materials and Methods: During a 3-year period, a total of 1715 endoscopies were performed in our pediatric endoscopy unit; 182 (10.6%) of them were performed for UGB to 158 patients. Fifty-six of them had emergent endoscopy. Among them, 15 cases with nonvariceal UGB were only given endoscopic hemoclips. Demographic, clinical, and laboratory findings at initial admission; endoscopic appearance of bleeding lesions; and outcome of hemoclip application were recorded from the hospital files and endoscopy records.Results: Ten patients (66.6%) had gastric ulcer, 3 (20%) had duodenal ulcer, 1 (6.7%) had Dieulafoy lesion, and 1 (6.7%) had bleeding at the post-polypectomy site. Initial homeostasis after hemoclip application was achieved in all patients (100%). Rebleeding was seen in only one patient (6.5%) with a Dieulafoy lesion, who needed hemoclip application for a second time, and the bleeding was controlled successfully. Permanent hemostasis was 100%. the median number of hemoclips used per case and per application was 3.4 and 3.2, respectively. None of the patients experienced any complication related to hemoclip application. Median duration of hospitalization was 6 days. on follow-up, none of the patients received surgical therapy, and 30-d mortality related to bleeding was 0%.Conclusion: the use of hemoclips for nonvariceal UGB in children is an effective modality to control bleeding without any complications in children.

    Effects of Chronic Gastritis on Sleep and Quality of Life in Adolescents

    Aim: This study aims to evaluate the sleep quality and the quality of life (QoL) of adolescents with chronic gastritis, and determine the related factors. Materials and Methods: This study included patients who were diagnosed with chronic gastritis both clinically and histopathologically. the Pittsburgh Sleep Quality Index and Epworth Sleepiness scale were used to assess sleep quality, and the Pediatric Quality of Life Inventory was used to assess QoL. the control group included healthy volunteers with no chronic diseases and no gastrointestinal system complaints. Results: Fifty-six patients with gastritis and 55 healthy volunteers were included. the patients with gastritis were found to have significantly lower overall QoL subscale, and total scale scores, except for the social functioning total score. Except for the subjective sleep quality, significant differences were seen between the groups in the sleep quality subscale, total scale, and sleepiness scale averages. the total score of the sleep quality scale was significantly higher in patients who had lower incomes than expenditure. There were no significant differences between the obese or overweight patients with gastritis and the rest of the gastritis patients in the study sample. There were no significant differences found between the Helicobacter pylori positive and negative gastritis patients in terms of the total QoL and sleepiness scale scores. Conclusion: This study is important because it is the first study in adolescents in this respect. the sleep and QoL scores of those adolescents with chronic gastritis were lower than the control group.

    Giant cell hepatitis and autoimmune hemolytic anemia after chickenpox

    Otoimmün hemolitik anemi ile dev hücreli hepatit çocuklarda ayrı bir klinik tablodur. Bu hastalık genellikle ilerleyici karaciğer hastalığı ile ölümcül seyreder. Geleneksel ilaçlarla yapılan immünsüpresif tedaviye kısmi yanıt sağlar, bununla beraber bu yanıt genellikle geçicidir. Monoklonal antikorlar ile yapılan altenatif tedavi sonrası hastalığın remisyonu rapor edilmiştir. Biz suçiçeği infeksiyonu sonrası otoimmün hemolitik anemi ve dev hücreli hepatit tanısı alan, standart immunsüpresif ve rituksimab tedavisine dirençli bir kız hastayı rapor ediyoruz. Bu çocuk hastada tedaviye rağmen remisyon sağlanamadı ve septik komplikasyondan hasta kaybedildi.

    Lymphedema treatment in a patient with a history of intestinal transplantation and mesenchymal stem cell transplantation

    Transplant hastalarında sirolimusa bağlı lenfödem gelişimi, nadir görülen bir komplikasyondur. Burada, sirolimus tedavisi sonrası üç ekstremitesinde lenfödem gelişen 14 yaşında kız hasta sunulmuştur. Ateşli silah yaralanması sonrası superior mezenterik arter, ileum ve kolon yaralanması nedeniyle, ileum ve kolon rezeksiyonu yapılan ve kısa barsak sendromu gelişen hastaya, yaralanmadan 4 ay sonra kadavradan ince barsak transplantasyonu, rejeksiyonu önlemek için mezenkimal kök hücre nakli uygulanmıştır. Postoperatif 3. ayda akut böbrek yetmezliği gelişen hastanın kullandığı mikofenolatmofetil tedavisi kesilerek sirolimus tedavisine geçilmiştir. Postoperatif 11. aya kadar mevcut tedavisiyle izlenen hastanın her iki bacağında ve sol kolunda şişlik gelişmiştir. Kompleks dekonjestif tedavi ile hastada anlamlı iyileşme gözlenmiştir. Bu olgu, ince barsak transplantasyonunda mezenkimal kök hücre nakli yapılan literatürdeki ilk olgu olup, üç ekstremitede lenfödem tutulumu olması ve lenfödem tedavisi uygulanması açısından farklılık göstermektedir.

    The Role of Familial Mediterranean Fever Gene Mutation in Treatment of Infantile Colitis With Resistant Perianal Fistula

    ABSTRACT Symptoms of infantile inflammatory bowel disease (I-IBD) can be life-threatening and associated with poor prognosis. the presence of Mediterranean fever (MEFV) gene mutations play an important role in treatment of I-IBD. in this article, we describe a case of I-IBD with a resistant fistula, in which remission occurred following colchicine therapy. the patient was a six-month-girl with complaints of bloody diarrhea and a perianal abscess of three months duration. Laboratory tests revealed elevated inflammatory parameters, hypoalbuminemia, and anemia. Results of repeated viral, bacterial and parasitic analyses were negative. Endoscopic and histopathological examinations confirmed a diagnosis of I-IBD. Although diarrhea episodes decreased following intensive conventional treatment with immunosuppressive therapy and anti-tumor necrosis factor, the perianal abscess and fistula did not resolve. Molecular genetic analysis to identify causes of infantile disease revealed the MEFV gene mutation. Thus, colchicine was added to the treatment regimen. Following treatment with colchicine, defecation returned to normal, and the fistula resolved. the MEFV gene mutation should be investigated in children with infantile colitis and resistant fistulas, particularly in Mediterranean countries. in patients with infantile colitis who have the MEFV gene mutation, colchicine treatment may be an alternative to intensive immunosuppressive therapy.

    The subclinical effect of celiac disease on the heart and the effect of gluten-free diet on cardiac functions

Turk J Pediatr 2016; 58: 241-245.This study aimed to investigate the effects of celiac disease (CD) on cardiac function in children by using conventional transthoracic echocardiography (TTE) and tissue Doppler echocardiography (TDE). Forty-nine patients diagnosed with CD were enrolled into the study. Group 1 consisted of 26 (53%) patients who had recently been diagnosed and had not been on glutenfree diet whereas Group 2 consisted of 23 (47%) patients who had been on regular gluten-free diet for at least 10 months. 20 healthy children were enrolled into the study as the control group. the deceleration time (DT) and the left ventricle (LV) isovolumetric relaxation time (IVRT) were significantly shorter in Group 1 compared to Group 2 and the control group (p=0.002, p=0.015). Mitral valve E/E' ratio was significantly lower in Group 1 and 2 when compared to the control group (p<0.001). the study demonstrated that evaluation of these parameters by using TDE could be beneficial in the early diagnosis of cardiac involvement in children with CD.
