Usage of vitreous humour hypoxanthine and potassium values for the estimation of postmortem interval

Our study has evaluated the first 37.5 hour postmortem concentrations of hypoxanthine (Hx), potassium (K+), chloride (Cl-), and sodium (Na+) in vitreous humor (VH) samples retrieved from 65 cases (whose postmortem time intervals (PMIs) and causes of death were known) between April 28. 2004 and August 17. 2004 at the Institute of Forensic Medicine, Ankara Group Chairmanship, Morgue Department. Although in many studies performed for the chemical analysis of VH till today it has been stated that the most valuable known method to estimate PMI(Postmortem interval) especially after the first 24 hours of postmortem period is utilization of K+ values. Some researchers pointed out that usage of Hx in the first 24 hours of postmortem period may give better results for the determination of the postmortem time interval. Our study has been planned to assess the usability of these parameters for the determination of the postmortem time interval. In our study, we found significant correlation between postmortem VH Hx and K+ values obtained from both eyes (r = 0.646, p < 0.001). Hx concentration increased in a linear manner and this increase has been started immediately after immediate postmortem period, not after establishment of a stable period. where K+ concentrations have shown a better correlation with the postmortem interval. Yet, within the first 37.5 hours after time of death, using the formulae developed by Munoz, the estimation of postmortem time interval calculated with Hx values gives more realistic results compared with K+ values estimates. In addition, the formulae developed by Munoz give more realistic results compared with the formulae proposed by Rognum and Madea for the determination of postmortem time interval. The samples from VHs of the right and left eyes have been collected simultaneously, and examination of postmortem VH Hx, K+, Cl- and Na+ concentrations did not demonstrate statistically significant difference between both eyes. Although increase in the ambient temperature caused a marked increase in Hx concentrations, it has been observed that effect of the ambient temperature on increases in K+ concentrations was at a minimal level. No significant linear relationship between postmortem vitreous humor Cl-, Na+ concentrations and postmortem interval has been determined, and hence utilization of these parameters in the determination of postmortem time may give erroneous results. Together with the methods used in forensic practice, we conclude that the utilization of postmortem vitreous humor concentrations of Hx and K+ may give realistic results about the time of death.
