In Eastern Mediterranean patients who apply for rhinoplasty, the patients' concerns should be very thoroughly listened and the goals should be discussed with the patient. Three major complaints are dorsal hump, droopy, and wide tip. The surgeon should be able to use both endonasal and external approaches in these patients. Dorsal hump removal should be done as meticulously as possible by using traditional and powered instruments. Middle vault reconstruction should be performed by spreader flaps or grafts. In selected cases, dorsum preservation should be mastered as well. In tip plasty, lateral crural turn-in flap is a good solution in wide or boxy tips. Tip sutures should be the mainstay in primary rhinoplasty, whereas the grafts should be more elaborately used in revision cases. The nasal septum should be addressed from simpler septoplasty to difficult segmental reconstruction techniques. Revision rhinoplasty should be performed by experienced surgeons. In cases where grafts are needed, oblique-split sculpting method of the rib cartilage is a game changer. © The Author(s), under exclusive license to Springer Nature Switzerland AG 2022.
