    Complement activation during cardiopulmonary bypass and effect of preoperative corticosteroid administration

    The efficacy of two widely used corticosteroids, dexamethasone (DM-Group I) (1 mg/kg) and methylprednisolone (MP-Group II) (30 mg/kg), in inhibition of complement activation and related damaging effects of neutrophil (PMN) response during cardiopulmonary bypass was studied in 37 patients (10 female) randomised in three groups, one served as control. With the initiation of CPB, complement system was activated in all three groups as evidenced with a decrease in plasma levels of C3 (p<0.01). In all groups, no further C3 activation was observed throughout CPB. However, at the termination, C3 component was found to be activated and decreased. Mean plasma levels of C3 were high in MP group (at the 10 min.: p<0.01, 30 min.: p<0.05, and the termination of CBP: p<0.01) compared with the controls while there was no significant difference in DM group (p>0.05). At the termination of CPB, transpulmonary leukocyte sequestration in 3 groups was found low. Even though the sequestrations were lower in the corticosteroid treated groups than the controls, the difference was not significant among the groups (p>0.05). It was found that C4 levels showed no change during CPB, and decreases in plasma levels of C4 after protamine infusion demonstrated an activation via the classical pathway and this activation was less with the MP group (Group I: p<0.01, Group II: p<0.05, Group III: p<0.01). After CPB, no sign of decrease demonstrating peripheral leukopenia was observed in MP and DM groups but the difference was significant in the control (p<0.05). In conclusion, prebypass treatment with both corticosteroids failed to inhibit the complement activation during CPB but MP treatment seemed to lower the degree of the activation.
