Gremlin-1 has roles in both pituitary tumorigenesis and angiogenesis. Therefore, we aimed to detect retinal micro-vascular changes in acromegaly with OCTA and examine its association with serum Gremlin-1 levels. Design: A cross-sectional, case-control study Methods: We included 53 acromegaly patients and age/gender-matched 31 healthy controls. To evaluate retinal structure, optical coherence tomography angiography (OCTA) imaging was performed. Gremlin -1 levels were measured by the ELISA method. Results: Acromegaly patients had significantly lower superficial total density (p < 0.001), superficial parafoveal density (p = 0.002), superficial perifoveal density (p < 0.001); deep total density (p < 0.001), deep parafoveal density (p < 0.001), deep perifoveal density (p < 0.001). Total thickness and perifoveal thickness were significantly lower in the acromegaly group. Gremlin-1 levels were significantly lower in acromegaly patients (1.22(0.13) vs 1.38(0.29); p = 0.015, for acromegaly and control groups, respectively). We assessed study groups based on glucose tolerance status. Normal glucose tolerance (NGT) acromegaly patients had significantly lower superficial total density (p = 0.036), superficial perifoveal density (p = 0.003), deep total density (p = 0.023), and deep perifoveal density (p = 0.023) compared to NGT controls. According to backward linear regression analysis, the presence of acromegaly itself consistently showed a significant negative impact on all types of vascular density. Conclusion: Patients with acromegaly have decreased vascular density and lower levels of Gremlin-1 independent of glucose tolerance status. Acromegaly may cause a reduction in gremlin-1 as a compensatory mechanism due to high IGF-1 levels known as an angiogenic factor, which in turn leads to the decrease in vascular density, or gremlin-1 may already have shown a decline in response to chronic inflammation and endothelial dysfunction in acromegaly resulting in a reduction in vascular density.Öğe Clinically correlated dose of the amniotic membrane extract is superior to its transplantation in corneal wound healing(Molecular Vision, 2024) Korkmaz, Ilayda; Kocamanoglu, Meltem; Gurdal, Mehmet; Arici, Mesut; Yaman, Banu; Palamar, Melis; Egrilmez, SaitPurpose: This study investigates the superiority of sterile lyophilized amniotic membrane extract (AME) prepared at a clinically correlated dose over amniotic membrane transplantation (AMT) in an experimental corneal wound model. Methods: AME was prepared from a pool of five amniotic membranes. After homogenizing the membranes, they were lyophilized and sterilized by gamma radiation to obtain sterile, lyophilized AME powder. The total protein amount and growth factor levels were measured in the AME samples. AME eye drops were prepared considering the protein concentration of the standard-size amniotic membrane weight used for transplantation, and this total amount was used as the daily dose. For the experimental animal corneal wound model, a full-thickness mechanical corneal epithelial defect was created in 15 eyes of 15 New Zealand rabbits. The rabbits were divided into four groups: Group 1: AME eye drop (n = 4 eyes), Group 2: AMT (n = 4 eyes), Group 3: preservation-free artificial tear (n = 4 eyes), and Group 4: control (n = 3 eyes). Daily anterior segment evaluation and photography were performed to determine the clinical efficacy of the AME. The rabbits were euthanized on day 7, and wound healing was examined histopathologically. Results: The total protein amount in the AME was 0.149 +/- 0.01 mg/ml. The growth factor levels were as follows: EGF = 41.19, FGF = 43.11, HGF = 203.67, KGF = 328.03, NGF = 207.92, and TGF-beta = 506.93 pg/ml AME. On clinical examination, the mean wound closure times in Groups 1, 3, and 4 were 2.75 +/- 0.50 (2-3), 3.5 +/- 1.0 (3-5), and 3.33 +/- 1.52 (2-5) days, respectively (p > 0.05). Histopathological examination revealed Group 1 corneal epithelium with full thickness, regular healing pattern, and normal anterior stromal keratocytes. In the remaining three groups, there were interruptions in epithelial healing, and loss of anterior stromal keratocytes was evident. Inflammation was more prominent in Group 2. Conclusions: AME is a liquid product that contains the essence of the amniotic membrane after homogenization and centrifugation. AME has the potential to overcome the disadvantages of AMT, such as surgery requirement and the limitation of postoperative objective clinical observation due to the semi-opaque nature of the amniotic membrane. Although, there are studies showing the advantages of AME over AMT in the literature, the preparation, preservation and sterilization of AME are still controversial. This study is specifically addressing the shortcomings of acquiring AME in the literature, such as minimizing inter-donor variability in AME by pooling amniotic membranes from different donors, lyophilizing AME to preserve its biochemical composition, and preventing infection transmission by using gamma sterilization. Herein, we observed that the AME prepared with this method contains high concentrations of growth factors. In the present study, the dose of AME was correlated with clinical use for the first time, and for the first time, the superiority of sterile lyophilized AME over AMT was clinically demonstrated in a corneal wound model. Furthermore, histopathological findings confirmed that AME seems to not only promote epithelial proliferation during wound healing but also prevent stromal keratocyte loss, inhibit inflammation and accelerate collagen remodeling.Öğe Nasal fluid sample as a reliable matrix for determination of cytokine levels in childhood asthma(Walter De Gruyter Gmbh, 2023) Doulatpanah, Mojtaba; Kocamanoglu, Meltem; Yıldırım Sözmen, Eser; Kartal Öztürk, Gökçen; Demir, Esen; Gülen, Figen; Akçay, YaseminObjectives: Childhood asthma is a chronic disease with high incidence worldwide. As a lifelong disease, asthma has episodes. Inflammation continues to occur in the clinical remission of asthma. It can be difficult to diagnose childhood asthma, especially in clinical remission. We hypothesized that some cytokines secreted to nasal fluid from the airway during inflammation might help diagnose clinical remission of asthma. Moreover, sampling nasal fluid is an easy and non-invasive procedure, so it may be a preferable sampling method.Methods: We measured levels of some interleukins (ILs), which are IL-4, IL-5, IL-6, IL-12p70, IL-13, IL-33, granulocyte-macrophage colony-stimulating factor (GM-CSF), periostin and thymic stromal lymphopoietin (TSLP) by Luminex magnetic bead-based immunoassay in nasal fluid and in serum of asthmatic children in clinical remission.Results: We found that IL-5, IL-6, IL-33, and periostin had elevated levels in nasal fluid. IL-5 and IL-33 had increased levels in the nasal fluid of the patients with immunoglobulin E (IgE) high and low phenotypes. While the nasal fluid TSLP levels were positively correlated with most of the increased serum cytokine levels of non-allergic asthmatic children, the nasal fluid GM-CSF levels were positively correlated with most of the increased serum cytokine levels of the allergic asthmatic children.Conclusions: IL-5, IL-6, IL-33, and periostin had elevated levels in the nasal fluid of the patients in clinical remission. The nasal fluid GM-CSF levels of the allergic patients and nasal fluid TSLP levels of the non-allergic patients had a positive correlation with most of the serum cytokine levels. Diagnosing ectopic pregnancy in the early period provides to reducing mortality and morbidity and gives an opportunity for medical treatment to preserve fallopian tubes. Evaluation of cervical fluid for determining ectopic pregnancy with new promising markers provided different aspects for diagnosing ectopic pregnancy in the present study. Methods in this prospective clinical study, ectopic pregnant patients as ectopic pregnancy group (n = 46), intrauterine pregnant patients as intrauterine pregnancy group (n = 29) and not-pregnant patients as nonpregnancy group (n = 10) participated to study. Cervical fluid samples were collected with using merocel sponge. in addition, serum samples were obtained from patients. Dynein heavy chain 5 (DNAH5) and creatine kinase (CK) levels were determined by enzyme-linked immunosorbent assay kits in samples. Results Reduced cervical fluid DNAH5 levels was diagnosed in ectopic pregnancy group compared to intrauterine pregnancy group (median 3.42 ng/mL; 25-75% percentile 0-9.56 ng/mL vs median 6.14 ng/mL; 1.40-8.31 ng/mL; P < 0.001). on the other hand, DNAH5 protein was not detected in nonpregnant patients' samples. in addition, statistical significant increased cervical fluid CK levels were diagnosed in ectopic pregnancy group compared to intrauterine pregnancy group (median 4477.61 IU/L; 0-64 925.37 IU/L vs 0 IU/L; 0-6832.30 IU/L; P = 0.006). Conclusion Measuring of CK and DNAH5 in cervical fluid could be promising markers for early diagnosing of ectopic pregnancy. Decreased DNAH5 levels in cervical fluid might be result from abnormal cilia function in ectopic pregnant patients. ClinicalTrials.gov ID.
