    Complement consumption during cardiopulmonary bypass: comparison of Duraflo II heparin-coated and uncoated circuits in fully heparinized patients

    Heparin attachment to synthetic surfaces is one means of improving the biocompatibility of clinically used cardiopulmonary bypass (CPB) circuits. To assess the effect of heparin-coated circuits on complement consumption during CPB, 40 patients undergoing elective myocardial revascularization were prospectively randomized either to a group in which a completely Duraflo II heparin-coated circuit was used for perfusion (heparin-coated Group, n = 20 patients) or to a control group (n = 20 patients) in which an uncoated, but otherwise standard circuit was used. Full systemic heparinization was induced (activated clotting time, 480 seconds) in all the patients included in the study, regardless of which perfusion circuit was used. The two groups did not differ significantly in terms of bodyweight, aortic crossclamp and extracorporeal circulation times. No patient had difficulty in weaning from bypass and the postoperative period was uneventful in all patients. Concentrations of C3 and C4 were found to be within the 'normal' range in the prebypass period in both groups. There were no significant intergroup differences with regard to C3 and C4 consumption during CPB. We conclude that Duraflo II heparin-coated circuits have no effect in reducing complement consumption during CPB in fully heparinized patients.

    Effects of flow types in cardiopulmonary bypass on gastric intramucosal pH

    The aim of this study was to determine the relationship between splanchnic perfusion and oxygen consumption, and flow types in cardiopulmonary bypass (CPB), by measuring gastric intramucosal pH. Twenty patients undergoing elective open-heart surgery were prospectively randomized to receive either pulsatile or nonpulsatile flow during CPB. Gastric intramucosal pH was measured using gastric tonometry. A flowmeter was used to measure the inferior caval vein flow. A catheter was inserted through the femoral vein to sample blood from the iliac vein. Systemic vascular resistance index, gastric intramucosal pH, inferior caval vein flow and arterial, inferior vena caval and iliac venous blood gases were recorded at different times. Gastric intramucosal pH decreased in all patients; only in the nonpulsatile group was this decrease statistically significant. After 45 min of CPB, the pH was 7.37 +/- 0.03 compared with the prebypass value of 7.48 +/- 0.04 (p = 0.00016). After weaning from CPB, the pH was 7.358 +/- 0.02 compared with the prebypass value (p = 0.000037). At 2 h post-operatively the pH was 7.416 +/- 0.025 (p = 0.02). Systemic vascular resistance index rose in all patients during bypass in both groups. These changes did not have any statistical significances and after weaning from bypass returned to prebypass levels. We conclude that nonpulsatile flow in CPB is associated with reduced gastric intramucosal pH and the measurement of intramucosal pH during open-heart surgery provides important information about splanchnic perfusion.

    Effects of pleurotomy investigation on respiratory sequelae after internal mammary artery harvesting

    The preservation of pleural integrity during mammary artery harvesting may decrease atelectasis and pleural effusion during the postoperative period. We designed this retrospective study to evaluate the effects on postoperative pulmonary function of pleural integrity versus opened pleura, in patients who receive a left internal mammary artery graft. The study group consisted of 7, 141 patients who underwent elective coronary artery bypass grafting. The patients were retrospectively evaluated and divided into 2 groups: those who underwent internal mammary artery harvesting with opened pleura (n=873) or with pleural integrity (n=268). To monitor pleural effusion and atelectasis, chest radiography was performed routinely I day before operation and on the 2nd, 5th, and 7th postoperative days. The preoperative, after extubation, and 1st postoperative day values of partial oxygen pressure (PaO2) partial carbon dioxide pressure (PaCO2), and oxygen (O-2) saturation were recorded for comparison, as was the hematocrit. The mean age of the patients was 574 +/- 8.81 years. There were no significant differences between the groups in mean values of PaO2, PaCO2, O-2 saturation, and hematocrit after extubation or on the 1st postoperative day. Atelectasis on the 5th and 7th postoperative days, pleural effusion on the 2nd, 5th, and 7th days, and postoperative bleeding were significantly less in the group with preserved pleural integrity We showed that preservation of pleural integrity during internal mammary artery harvesting decreases postoperative bleeding, pleural effusion, and atelectasis. We conclude that preservation of pleural integrity when possible, can decrease these postoperative complications of coronary artery bypass grafting.

    Endovascular Stent Graft Placement in the Treatment of Ruptured Tuberculous Pseudoaneurysm of the Descending Thoracic Aorta: Case Report and Review of the Literature

    We report a successful repair of a ruptured tuberculous pseudoaneurysm of the descending thoracic aorta by endovascular stent graft placement. This procedure is starting to be accepted as an alternative method to surgery, and we review similar cases in the literature.

    Impact of organ malperfusion on mortality and morbidity in acute type A aortic dissections

    Background: Organ malperfusion is a serious complication of acute type A aortic dissection. Management and outcome of malperfusion has been discussed in this study. Methods: Between November 1994 and May 2003, 118 patients with acute type A aortic dissections were operated. Fifty-seven patients (48.3%) were complicated with organ malperfusion, which is considered as group I. Seventy-three ischemic events were seen in 57 patients with organ malperfusion. Patients in group I were divided into four subgroups according to affected organ system including limb (38 events), coronary (9 events), renal (2 events), visceral (9 events), and cerebral (15 events) ischemia. Sixty-one patients without organ malperfusion constitute group II. Results: The hospital mortality rate was 42.1% (24 of 57) in patients with malperfusion, 14.75% (9 of 61) in group II (p = 0.001), and 27.9% (33 of 118) in all patients. Postoperative complications such as mediastinal hemorrhage, low cardiac output, gastrointestinal system complications, acute renal failure, and multiple organ failure were higher in group I. Mesenteric and limb ischemia associated with high mortality. Multivariate analysis reveals that visceral malperfusion is the strongest predictor of postoperative mortality (odds ratio: 25.09, p = 0.000). Isolated coronary malperfusion had the lowest mortality (one patient, 16.6%) among the patients with organ malperfusion. Conclusions: Acute type A aortic dissections with organ malperfusion has higher postoperative mortality and morbidity. Immediate aortic repair is our management strategy in patients with limb, coronary, and neurological malperfusion. To reduce the extremely high mortality with mesenteric malperfusion, new strategies should be investigated such as surgical delay with interventional procedures.
