Öğe The effects of morphine and fentanyl alone or in combination added to intrathecal bupivacaine in spinal anesthesia for cesarean section(2011) Karaman S.; Günüsen I.; Uyar M.; Biricik E.; Firat V.Objectives: This randomized double-blind controlled trial examined the efFects of fentanyl and morphine, alone and in combination, as adjuncts to spinal anesthesia for elective cesarean section. Methods: Sixty women undergoing elective cesarean section, with spinal anesthesia using 0.5% hyperbaric bupivacaine, were randomly allocated to receive morphine 0.2 mg, fentanyl 25 µg, or fentanyl 12.5 µg plus morphine 0.1 mg, intrathecally. The start of spinal block, the time to TIO level, the highest sensorial and motor block level, time to regression of sensory block to TIO, time to resolution of motor block, surgical characteristics, maternal side efFects, Apgar and NACS scores, umbilical blood gas evaluations, and time to first analgesic requirement were recorded. Results: No patient experienced pain during the intraoperative period. The degree and time of sensorial and motor block were similar in both groups, and there was no difference in time to TlO level and time to reversal of motor block. The difference in time to first postoperative analgesic requirement was statistically significant. There was no difference between groups in postoperative side efFects. There were no neonatal differences in Apgar and NACS scores or umbilical blood gas evaluations. Conclusion: The quality of postoperative analgesia with morphine, when used alone, was found to be superior to that with fentanyl. The combination of opioids offered no advantages over morphine alone.Öğe Evaluation of symptoms and clinical features associated with fibromyalgia syndrome(Anestezi Dergisi, 2018) Biricik E.; Erhan E.; Pirim A.; Yegul I.Objective: Fibromyalgia syndrome is clinical state that is common with symptoms and signs may affect quality of life. In this study, we aimed to investigate symptoms and findings associated with fibromyalgia. Method: Patients who admitted to Pain Clinic between 2003-2004 previously diagnosed fibromyalgia were evaluated prospectively for physical examination, anxiety, depression and clinical complaints. The Hamilton depression scale and state anxiety scale were used. Results: A total of 86 patients admitted to clinic, but 8 patients were out and study was completed with 80 patients. Seventy-seven patients were female (96.3%), 3 were male (3.7%). All but one of patients had generalized pain complaints, 63% had regular analgesics (78.7%). Fourteen patients had antidepressant use. Anxiety score was measured 50.07±10.19 in patients using antidepressants, 52.79±12.1 in not taking. The anxiety score was measured 55.8±12.7 in patients using analgesics, 51.35±11.4 in patients without analgesics. The highest anxiety scores were observed in 60-69 age. The severity of common pain increased with anxiety scores. There was no depression was observed in 42 patients (42.5%). Mild depression was found in 29 patients (36.2%), moderate in 8 (10%), severe in 1 (1.2%). Conclusion: Fibromyalgia is a common syndrome characterized by pain and associated with anxiety and depression. © 2018 Anestezi Dergisi. Although hysteroscopy is a safe and short procedure, particularly operative hysteroscopy brings along many risks that anesthetists should consider about. In this report, we presented two cases in which pulmonary edema, hyponatremia and venous air embolism developed during the operative hysteroscopy, and aimed to draw attention to the management of these complications.
