Material and Method: We performed a questionairre on 202 residents who studied general surgery residency in 9 state universities in Ankara, Istanbul, Izmir and Bursa. The residents were asked about which medical school they have graduated from, how many times they joined residency examination, in which examination they succeede to start residency and the order of their choice of general surgery residency was in their list. Results: Among those 202 residents, only 40 of them preferred general surgery residency as the first choice in their list (19.8 %). Most of residents who were successful in the first examination period preferred Ankara University General Surgery Department (44.4%). The residents who preferred general surgery residency as the first choice were found mostly in Hacettepe University General Surgery Department (37.5 %). Most residents who preferred general surgery residency in their first 3 choice in their list were found in Uludag University General Surgery Department (64.3 %). Examination for medical doctor residency April 2009 results have shown that general surgery residency was chosen in the 24th level among 25 branches. Conclusion: As a result general surgery residency was found to be placed at the end of the choices in residency examination among other branches in our trial.Öğe Double cystic duodenal duplication: A case report and review of the literature(1993) Balik E.; Taneli C.Duplications of duodenum are relatively rare congenital abnormalities of the gastrointestinal tract. Although duplications can occur anywhere in the alimentary tract, duodenal duplications are encountered only 5-10% of the total. Up to date, only a single case of double cystic duplication is reported in the literature. A new case of this very uncommon abnormality is present.Öğe Öğe Hypertonic saline injection in the treatment of rectal prolapsus: An experimental and clinical study(1993) Taneli C.; Ilhan H.; Ozcan C.; Tuncyurek M.; Balik E.Hypertonic saline injection is one of the methods used for the treatment of rectal prolapsus. Saline solution in 30% has been frequently preferred. Severe inflammatory reaction and perirectal abscess formation are the complications most often encountered. To identify the ideal concentration of the perirectally injected hypertonic saline, 28 rabbits were put into 4 groups and were given injections of a single dose of either 5, 10 or 30% of saline solutions to the study group and isotonic saline to the control group. After a period of 4 weeks the anorectal segments were extirpated and studied histopathologically in view of perirectal fibrosis. Fibrosis was encountered in all groups but was most pronounced in the group injected 30% saline. Post injection complications were seen only in the group of 30% saline injections. Following the present experimental study a clinical study was started this time in 14 patients with rectal prolapsus. In 7 of the study group 5% saline injections, in 7 of them 10% saline injections were performed. In conclusion; 5% saline injections were found ineffective. Although 30% saline injections were effective enough, the risk of complication was high. Whereas in the group of 10% injections the concentration of saline was effective to form an adequate amount of fibrosis without any risk of complication.Öğe The incidence of inguinal complications after ventriculoperitoneal shunt for hydrocephalus(2005) Çelik A.; Ergün O.; Arda M.S.; Yurtseven T.; Erşahin Y.; Balik E.Aim: This study outlines the incidence of acquired clinical inguinal manifestations (CIM) in infants undergoing ventriculoperitoneal (VP) shunt procedures, and the possible underlying mechanisms in the development of these pathologies. Material and methods: The charts of c hildren who had undergone VP shunt procedures for hydrocephalus between 1992 and 2002 were reviewed. All patients were contacted by a telephone interview, and they were invited to be examined in the outpatient clinic of paediatric surgery for the development of groin manifestation. Results: Eighty-eight patients resp onded to the inquiry. Twenty-one (23.8%) patients developed a CIM. Three of them were premature infants. The mean interval between the shunt procedure and the diagnosis of CIM was 5.3 months (20 days to 48 months). Nineteen of the patients who developed a CIM were in the 0- to12-month age group when the VP shunt procedures were initially performed. Ten CIM occurred on the right (47%), 7 on the left (33%) and 4 were bilateral (20%). The rate of bilateral patency as an operative finding was 76%. There was one recurrence (4.8%). Conclusion: The younger the age at which the VP shunt was performed, the higher the incidence of CIM. Bilateral manifestations were higher than in the normal population. Increased intra-abdominal pressure by accumulation of fluid in combination with the high patency rate of processus vaginalis possibly plays a role in the development of CIM in VP shunt patients. Bilateral repair should be considered even when CIM is unilateral. All rights reserved.Öğe Öğe Ovarian tumors in children [Çocuklarda over tümörleri](2005) Çelik A.; Tanriverdi H.I.; Kantar M.; Özcan C.; Çetingül N.; Balik E.Aim: Tumors of the ovaries are uncommon in children. Most of them are benign and in germ cell origin, opposite to adults. In this study, children with ovarian tumors were rewieved in respect to their diagnosis, treatment and results. Method: The hospital records of 30 patients (33 ovaries) with ovarian tumors treated at our department between 1984 and 2004 were reviewed retrospectively. Results: The mean age was 10.7±3.43 (range; 3 to 18 years) years. The tumors were on the left, right and bilateral in 16, 11 and 3 patients, respectively. The histologic diagnosis of the tumors were mature teratoma (n=18), immature teratoma (n=1), dysgerminoma (n=8), mixed germ cell tumor (n=1), gonadoblastoma (n=1), serous cystadenoma (n=1), mutinous cystadenoma (n=2) and granulosa cell tumor (n=1). Abdominal pain (n=18), abdominal distention (n=11) and vomiting (n=9) were the most common symptoms. Six patients had acute abdominal findings requiring urgent surgery. The surgical procedures were salpingooophorectomy (n=21), oophorectomy (n=6), and total tumor excision with ovarian salvage (n=6). In malignant ovarian tumor group (n=11); median fellow-up period was 84.2±47.14 months with a range of 8 to 144 months. One patient with Stage 4 tumor died 6 years later. One patient lost to follow-up in the malignant group. In two patients with diagnosis of mature cystic teratoma a new tumor was determined in the contralateral ovary with a similar histology after first operation 6th and 55 th months, respectively. Conclusion: The majority of the patients had germ cell tumors in this series, and surgical treatment was all that was required for complete cure in most of them. The adnexial pathologies because of their devastating complications should be taken into consideration in differential diagnosis in young females presenting with acute or chronic gastrointestinal symptoms.Öğe The Rapunzel Syndrome: A Case Report and Review of the Literature(1993) Balik E.; Ulman İ.; Taneli C.; Demircan M.The Rapunzel syndrome, found characteristically in girls with varying gastrointestinal symptoms, is a rare form of gastric trichobezoar extending throughout the bowel. All rights reserved.Öğe Removal of open safety pins from the stomach and duodenum of infants without enterotomy(Springer-Verlag, 1995) Balik E.; Avanoglu A.; Ulman I.; Erickçi V.; Demircan M.Twelve infants aged 6-18 months with ingested open safety pins (OSPs) were operated upon between 1984 and 1993. The OSPs were wide-open in 4 and the retention time was more than 3 days in 8. At laparotomy, the stomach and duodenum were explored and the OSP and a Levin tube inserted previously from mouth to stomach were identified by palpation. The pin was inserted through the holes of the tube and locked intraluminally without opening the stomach and the tube was then withdrawn from the mouth with the OSP at its tip. When the OSP was located in the duodenum, it was squeezed back into stomach for removal. If the OSP could not be squeezed back into the stomach due to an unfavorable position, it was locked intraluminally and milked distally for spontaneous discharge. No complications related to the procedure were encountered. In the literature cases are usually diagnosed at operation and it is noted that in former years splenectomy had a special place among various methods of treatment. In this report, a 7-year-old case of splenoptosis diagnosed on ultrasonography and isotope scintigraphic methods preoperatively and treated by splenopexy is presented. All rights reserved.Öğe Thoracoscopic procedures in children with anterior mediastinal tumors [Çocuklarda ön mediasten tümörlerinde torakoskopik girişimler](2005) Özcan C.; Çelik A.; Arda S.; Balik E.Aim: Thoracoscopy is frequently performed in pediatric patients for the diagnosis or treatment of mediastinal masses. In this study, we present our experience with thoracoscopic surgery in patients with anterior mediastinal tumors. Method: A total of 7 thoracoscopic procedures were performed in 6 children. Mean age of patients was 11.3 (7-14) years. Selective intubation of contralateral main bronchus, obliteration of ipsilateral main bronchus by a Fogarty catheter or CO2 insufflation into the pleural space with 5-8 mmHg pressure were the methods used to collapse the ipsilateral lung. Three trocars and 5 mm 0-degree telescope were used in all procedures. After 6 procedures, a chest tube was inserted through one of the trocar tracts while no drain was used after 1 procedure. Results: The biopsies (4 insicional, 2 excisional) were performed for a primary histologic diagnosis in 4 children and for the assessment of residual masses persisted after therapy in the remaining 2 patients. A second thoracoscopy and total resection of the mass was performed in a patient biopsied previously, and whose tumour shriked but not completely disappeared after chemotherapy. Conversion to thoracotomy was not required in any of the procedures and no postoperative complication was observed. The mean number of chest tube days was 1.6 (1-3). The histologic diagnosis of the tumors biopsied for a primary diagnosis were undifferentiated small round cell malign tumor (n=1) and Hodgkin Lymphoma (n=3). The pathologic examination revealed benign histology in residual tumors persisted after cessation of therapy (n=3). Conclusion: Thoracoscopic surgery in the treatment of children with anterior mediastinal tumors can be efficacious and safe. Mediastinal masses can be biopsied and resected by thoracoscopy alone.Öğe Thyroglossal duct remnants - Infrahyoid extension as a cause of recurrence(Springer-Verlag, 1995) Balik E.; Sayan A.; Mevsim A.; Ulman I.; Taneli C.Ten children were admitted with recurrences of thyroglossal duct remnants (TDRs) during the years 1978-1993. Unexcised infrahyoid extension of the thyroglossal duct was found to be the cause of recurrence in 3 cases, whereas the remainder were due to misdiagnosis and inappropriate surgery. After surgical treatment, these three cases were evaluated regarding the cause of recurrence. The treatment of choice for these lesions is the Sistrunk procedure. The complete Sistrunk procedure solves the problem in the majority of patients, but in cases with infrahyoid extension tracing of the extension is required. It is concluded that an appropriately performed Sistrunk procedure assures a complete cure in the vast majority of cases of TDRs; however, routine exploration for and excision of the occasional infrahyoid extensions is recommended for the prevention of recurrence. © 1995 Springer-Verlag.
