    Effects of N-acetylcysteine treatment on oxidative stress in acetic acid-induced experimental colitis in rats

    We assessed the possible protective effects of N-acetylcysteine (NAC) against toxic damage in the rat colon. Two doses of NAC (20 mg/kg and 100 mg/kg) given for 2 days and 7 days after acetic acid administration (to induce colitis) were tested. NAC was dissolved in saline and administered locally (intracolonic), systemically (intraperitoneal) or in a combination (intracolonic and intraperitoneal). Several parameters, including macroscopic and histopathological scores and myeloperoxidase, glutathione and nitric oxide concentrations were measured using standard assay procedures. Treatment with 100 mg/kg NAC for 7 days significantly decreased tissue myeloperoxidase, glutathione and nitric oxide concentrations. The 20 mg/kg dose had no protective effects. The data indicate that NAC substantially reduced the degree of colonic injury, probably by regulating free radical production and inhibiting inflammation. It may, therefore, have a role in the treatment of inflammatory bowel disease. Copyright © 2005 Cambridge Medical Publications.

    Long-term outcomes of colon cancer patients undergoing standardized technique operation with curative intent

    There is no defined standard surgical technique accepted worldwide for colon cancer, especially on the extent of resection and lymphadenectomy, resulting in technical variations among surgeons. Nearly all analyses employ more than one surgeon, thus giving heterogeneous results on surgical treatment. This study aims to evaluate longterm follow-up results of colon cancer patients who were operated on by a single senior colorectal surgeon using a standardized technique with curative intent, and to compare these results with the literature. A total of 269 consecutive patients who were operated on with standardized technique between January 2003 and June 2013 were enrolled in this study. Standardized technique means separation of the mesocolic fascia from the parietal plane with sharp dissection and ligation of the supplying vessels closely to their roots. Patients were assessed in terms of postoperative morbidity, mortality, disease recurrence, and survival. Operations were carried out with a 99.3% R0 resection rate and mean lymph node count of 17.7 nodes per patient. Surviving patients were followed up for a mean period of 57.8 months, and a total of 19.7% disease recurrence was recorded. Mean survival was 113.9 months. The 5-and 10-year survival rates were 78% and 75.8% for disease-free survival, 82.6% and 72.9% for overall survival, and 87.5% and 82.9% for cancer-specific survival, respectively. R1 resection and pathologic characteristics of the tumor were found to be the most important prognostic factors according to univariate and Cox regression analyses. Standardization of surgical therapy and a dedicated team are thought to make significant contributions to the improvement of prognosis.

    Posttransplantation malignancies and treatments [Organ transplantasyonu sonrasi gelisen maligniteler ve tedavileri]

    Long term immunosuppressive therapy following organ transplantation increases the risk of de novo malignant diseases development. Between 1985 through 1998, a total of 347 renal and 25 liver transplantations performed in Ege University Medical School Hospital, Department of General and Transplantation Surgery were studied retrospectively. During the follow up period, 15 patients with renal transplants and one with liver transplant have developed malignancy. In the management of the malignant diseases; reduction in the immunosuppression regimen for Kaposi's sarcoma, excision for epidermoid carcinoma in situ and basocellular carcinoma in situ, chemotherapy for lymphoma, conisation for cervix carcinoma in situ and modified radical mastectomy and adjuvant chemotherapy for breast carcinoma were performed. Mortality rate was %18.7(3 patients). Most of the malignancies after transplantation are skin related lesion and if they are diagnosed early are curable without organ loss. We suggest to screen all the patients for malignancy closer than the normal population, yearly mammograms, gynecologic examinations, occult blood and rectosigmoidoscopy and biopsy all skin lesion.

    Recurrent pilonidal sinus: Lay open or flap closure, does it differ?

    Treatment options of pilonidal sinus, which has high recurrence rates, is still controversial. In this study, we aimed to analyze for possible factors affecting recurrence. Forty-one patients with recurrent pilonidal sinus were included in this study. Of them, 33 were male and 9 were female (mean age, 24.9 years; age range, 16-42). Factors (i.e., risk factors) were detected in 32 patients. Excision-secondary healing and lay open was performed on 30 of the patients admitted with recurrence. Excision and flap closure was applied on 11 patients. Our recurrence rate was 9.7%. The recurrence rate of our study is compatible with the literature. Comparative studies are needed to determine the appropriate method to decrease recurrence rate.

    Steroid utilization in eosinophilic jejunitis: Beneficial or harmful?

    Purpose: Eosinophilic jejunitis is a rare disorder of undetermined origin, which is characterized by infiltration of eosinophils in the intestine. The aim of this study is to evaluate steroid therapy effect in patient treated for eosinophilic jejunitis to share our experience with other colleagues. Methods: We report a patient with symptoms of small bowel obstruction whose diagnosis was confirmed by previous operation owing to a well-known pathology due to an unusual reason, perforation, which was masqueraded by therapeutic drugs. Result: This clinical entity is usually encountered between the third and fifth decades. Stomach and small intestine are the most affected organs; however, it can affect any part of the gastrointestinal system. Abdominal pain and obstructive symptoms concomitant with weight loss are the common presenting complaints. According to current knowledge, avoiding unnecessary resection unless it is inevitable is an acceptable option for the management of eosinophilic jejunitis disease. Conclusion: Recent data suggest that eosinophils may directly damage the gastrointestinal tract wall in eosinophilic gastroenteritis. A few limited literatures about this disease strongly recommends steroid therapy for treatment. Furthermore, beneficial or harmful effects of steroids in eosinophilic jejunitis and its treatment details are discussed and reviewed. © 2009 Springer-Verlag.

    Radiopharmaceuticals are radioactive medications (radioisotopes) and are composed of radionuclidic and pharmaceutical parts. Recently, the use of radiopharmaceuticals as diagnostic and therapeutic agents is increasing. several approaches have been employed to develop therapeutic radiopharmaceuticals. Therapeutic radiopharmaceuticals have essential roles in nuclear medicine administrations. today, various diseases such as thyroid cancer, metastatic bone cancer, neuroendocrine tumors, and myeloproliferative can be treated with radioimmunotherapy. These treatments provide convenience in multiple ways and can be advantageous compared to other treatment methods. in this review, current radiopharmaceuticals and their usage in different disease treatments are summarized by providing fine details.     Unexpected Benefit of the Positron Emission Tomography/Computed Tomography Using 18F-Fluorodeoxyglucose: Report of a Case

    The positron emission tomography/computed tomography (PET/CT) has been a new tool utilized in the diagnosis and staging of various cancers. However, common worldwide utilization of the PET/CT includes some economic, legal, and ethic controversies. Although PET/CT scanning can detect colorectal premalignant lesions in an early treatable stage, most governments’ health care system does not pay for it as a screening test because of its economic burden. Thus, people are forced to make vital decisions about their health because of health policies of their governments. Here, we present an unusual case and discuss the utilization of PET/CT for detection of incidental neoplasms. © 2016, Association of Surgeons of India.
