Ostomy care nurses have a crucial role in the assessment and management of peristomal skin disorders, and spend a significant proportion of their time preventing and managing them The Ostomy Skin Tool is valid and reliable tool. The Ostomy Skin Tool provides a common language to describe the severity, extent and likely cause of a peristomal skin disorder. The Ostomy Skin Tool is comprised of two sections. Part one is used to calculate a score that describes the peristomal skin condition and incorporates both the area affected and severity of the problem. Part two is a diagnostic guide that allows categorisation of the peristomal skin disorder according to its cause and offers guidance on care.
