Özet
Giriş ve amaç Nöral Tüp Defektleri (NTD), embriyonun gelişim sürecinde.oluşan, çocuğu ve ailesini önemli ölçüde etkileyen doğumsal bir anomalidir. Anne adayının gebe kalmadan üç ay önce başlayarak hamileliğinin ilk üç ayına.kadar 0.4 mg/gün folik asit alması ile %70 oranında ödenebilmektedir. Bu nedenle kadınların bilgilendirilmesi ve NTD sıklığının azaltılması için öncelikle sağlık personelinin eğitilmesi hedeflenmiş ve bir eğitim programı hazırlanmıştır. Bu araştırmada eğitim programının başarısı ve etkinliği değerlendirilmektedir. Gereç ve Yöntem Toplumsal bir girişimin ilk aşaması olan bu araştırmaya 2006 yılında İzmir Narlıdere Sağlık Grup Başkanlığı Bölgesi'ndeki üç sağlık ocağında görevli toplam 51 sağlık çalışanı (21 hekim ve 33 ebe -hemşire) katılmıştır. Eğitimler ikisi hekimlere ve diğer ikisi ebe-hemşirelere yönelik dört oturumda verilmiştir. Bu eğitim oturumlarında eğitimin etkinliği ve başarısı uygulanan ön-son testlerle değerlendirilmiştir. Ayrıca 10 başlıkta beşli Likert skalası (5-çok iyi,...,1-kötü) kullanılarak katılımcı memnuniyeti belirlenmiştir. Bulgular Araştırmaya katılan hekimlerin meslekte çalışma yılı ortalaması 16.6±5.4, ebe-hemşirelerin ise 15.5± 4.94 yıldır. Her iki grupta da eğitimler ile NTD'leri ve spina bifida hakkındaki bilgi düzeylerinin anlamlı düzeyde arttığı belirlenmiştir (hekimler için t: 3,957, ebe-hemşireler için t: 8.142, p< 0.001). Hekim ve ebe-hemşirelerin eğitimlerden memnuniyeti beş puan üzerinden değerlendirildiğinde oldukça yüksek bulunmuştur (ortalama 4.80). Sonuç Önlenebilir hastalıklar için sağlık hizmetlerin sunulması önemlidir ve hem etik hem de ekonomik olarak önceliklidir. Sağlık personelinin bilgilerini güncelleyerek NTD sıklığının düşürülmesini hedefleyen bu eğitim programı oldukça etkilidir. Birinci basamakta ülke genelinde uygulanabilir özelliktedir.
Introduction and objective Neural Tube Defects (NTD) are congenital malformations, which occur during the development of the embryo and affect children and their families to a great extent. It has been known that folic acid prevents the occurrence of 70% of NTDs when a woman intending to become pregnant starts taking daily tablets containing 0.4 mg of folic acid three months before conception and continues throughout the first trimester of pregnancy. Thus, in order to inform women and decrease the frequency of NTDs, the training of healthcare workers was defined as a primary objective, which an educational programme was prepared. In this study, the success and efficiency of this educational programme is being evaluated. Method We started this study in 2006 as the first stage of a community-intervention research. 51 healthcare workers (21 physicians and 33 nurses/midwives) working in three primary care centers in the Narlidere Health District of Izmir participated. Two training sessions were given to nurses/midwives and two to physicians. The efficacy and success of these training sessions have been evaluated on the basis of pre and post tests. In addition, participant satisfaction was evaluated with 10 items on a 5-point scale (5: very good, ..., 1: bad). Findings In this research, the mean duration of working experience was 16.6±5.4 years for physicians and 15.5±4.94 for nurses/midwives. It was observed that in both groups, knowledge of NTDs and spina bifida improved significantly (for physicians t= 3.957, and for nurses/midwives t=8.142, p<0.001). When the views of the physicians and nurses/midwives about the given courses were evaluated on a 5-point basis, satisfaction with training sessions was found to be quite high (average 4.80). Results Giving health services for preventable diseases is important and has priority in terms of both ethics and economics. This educational programme aiming to lower the frequency of NTDs through updating the knowledge of healthcare workers has shown a satisfactory level of efficiency. It is also an applicable programme in primary care throughout the country.
