Özet
Aksiller Lenf Diseksiyonu Olan Meme Kanserli Hastalarda Lenf Ödem, Yorgunluk ve Yaşam Kalitesi Arasındaki İlişki Amaç: Tanımlayıcı ve kesitsel tipte olan bu çalışmanın amacı, aksiller lenf diseksiyonu olan meme kanserli hastalarda lenf ödem, yorgunluk ve yaşam kalitesi arasındaki ilişkinin incelenmesidir Yöntem: Tanımlayıcı ve kesitsel türdeki bu araştırma 1 Nisan 2022- 1 Temmuz 2022 tarihleri arasında Sağlık Bakanlığı Tepecik Eğitim ve Araştırma Hastanesi Onkoloji Merkezine başvuran aksiller lenf diseksiyonu olan meme kanserli 122 kadın hasta ile gerçekleştirildi. Verilerin toplanmasında; sosyodemografik verileri içeren "Birey Tanılama Formu", "Kanser Yorgunluk Ölçeği:", "EQ-5D Genel Yaşam Kalitesi Ölçeği" kullanıldı. Veriler, sayı, yüzde, ortalama, standart sapma, Mann Whitnye U Testi, Kruskal Wallis H Testi, Bonferroni Posthoc Testi, Spearman's Korelasyon ile analiz edildi. Bulgular: Araştırmaya katılan hastaların, 38,4'ünün 55 ve üstü yaş grubunda yer aldığı saptandı. Hastaların üst ekstremite lenfödem değerlendirme parametrelerinin sağ ile sol taraf arasındaki farkların ortalaması metakarpofalengeal için 0,83±1,13 cm, el bileği için 0,85±0,97 cm, bilek 10 cm üstü için 1,61±1,52 cm, dirsek 10 cm altı için 1,47±1,47 cm, dirsek 10 cm üstü için 1,97±1,76 cm ve gonyometrik ölçüm için 18,46±23,20 derece olarak bulundu. Hastaların EQ-5D Genel Yaşam Kalitesi puan ortalaması 7,34±2,00, EQ-5D Genel Sağlık Durumu puanortalaması 71,12±14,21 olarak bulundu. Hastaların Kanser Yorgunluk Ölçeği'nden aldıkları puan ortalamasının 35,86±6,98 olduğu saptandı. Hastaların EQ-5D Genel Yaşam Kalitesi puanı ile Fiziksel Yorgunluk alt boyutu (r=0,680) ve Kanser Yorgunluk Ölçeği toplam (r=0,624) puanları arasında pozitif yönde yüksek ilişki bulundu (p<0,05). Hastaların EQ-5D Genel Yaşam Kalitesi puanları ile Bilişsel Yorgunluk alt boyutu puanları arasında pozitif yönlü orta düzey ilişki saptandı (r=0,456, p<0,05). Hastaların EQ-5D Genel Yaşam Kalitesi puanları ile Duygusal Yorgunluk alt boyutu puanları arasında negatif yönlü orta düzey ilişki saptandı (r=-0,511, p<0,05). Hastaların EQ-5D Genel Sağlık Durumu puanları ile Fiziksel Yorgunluk alt boyutu (r=-0,685) ve Kanser Yorgunluk Ölçeği toplam (r=-0,626) puanları arasında negatif yönde yüksek ilişki bulundu (p<0,05). Hastaların EQ-5D Genel Sağlık Durumu puanları ile Bilişsel Yorgunluk alt boyutu puanları arasında negatif yönlü orta düzey ilişki saptandı (r=-0,417, p<0,05). Hastaların EQ-5D Genel Sağlık Durumu puanları ile Duygusal Yorgunluk alt boyutu puanları arasında pozitif yönlü orta düzey ilişki saptandı (r=0,469, p<0,05). Sonuç: Araştırmada hastaların yarıya yakınında lenfödem geliştiği, hastaların yaşam sağlık durumlarının yüksek, genel yaşam kalitelerinin ve yorgunluklarının orta düzeyde olduğu belirlendi.
Abstract The Relationship Between Lymph Edema, Fatigue, and Quality of Life in Breast Cancer Patients with Axillary Lymph Dissection Purpose: This descriptive and cross-sectional study is to examine the relationship between lymphedema, fatigue and quality of life in breast cancer patients with axillary lymph dissection. Methods: This descriptive and cross-sectional study was conducted with 122 female patients with breast cancer with axillary lymph dissection who applied to the Ministry of Health Tepecik Training and Research Hospital Oncology Center between 1 April 2022- 1 July 2022. In data collection; "Individual Identification Form" containing sociodemographic data, "Cancer Fatigue Scale:", "EQ-5D General Quality of Life Scale" were used. Data were analyzed with number, percentage, mean, standard deviation, Mann Whitnye U Test, Kruskal Wallis H Test, Bonferroni Posthoc Test, Spearman's Correlation. Results: It was determined that 28% of the patients participating in the study were in the age group of 44 and below, 33.6% were in the age group of 45 - 54, and 38.4 were in the age group of 55 and above. The mean of the differences between the right and left sides of the upper extremity lymphedema evaluation parameters of the patients was 0.83 ± 1.13 cm for the metacarpophalangeal, 0.85 ± 0.97 cm for the wrist, 1.61 ± 1.52 cm for the 10 cm above the wrist, It was found as 1.47 ± 1.47 cm for the elbow below 10 cm, 1.97 ± 1.76 cm for the elbow 10 cm above and 18.46 ± 23.20 degrees for the goniometric measurement. The mean EQ-5D General Quality of Life scores of the patients were 7.34 ± 2.00, and the mean EQ-5D General Health Status scores were 71.12 ± 14.21. The mean scores of the patients from the Cancer Fatigue Scale were found to be 35.86 ± 6.98. A high positive correlation was found between the patients' EQ-5D General Quality of Life scores and the Physical Fatigue sub-dimension (r=0.680) and Cancer Fatigue Scale total (r=0.624) scores (p<0.05). A moderate positive correlation was found between the patients' EQ-5D General Quality of Life scores and Cognitive Fatigue subscale scores (r=0.456, p<0.05). A moderate negative correlation was found between the patients' EQ 5D General Quality of Life scores and Emotional Fatigue sub-dimension scores (r=-0.511, p<0.05). A high negative correlation was found between the patients' EQ-5D General Health Status scores and the Physical Fatigue sub-dimension (r=-0.685) and Cancer Fatigue Scale total (r=-0.626) scores (p<0.05). A moderate negative correlation was found between the patients' EQ-5D General Health Status scores and Cognitive Fatigue sub-dimension scores (r=-0.417, p<0.05).
