Özet
ÖZET: UZUN SEGMENTLİ ÜRETER DEFEKTLERİNDE KULLANILMAK ÜZERE PERİTONDAN TÜP OLUŞTURULMASI Bir ön çalışma olan çalışmamızda üreteral devamlılığın sağlanabilmesi amacı ile bir stent etrafına omentum desteğinden yararlanarak peritondan tüp oluşturulması amaçlanmıştır. Gereci oluşturan 10 adet Yeni Zelanda türü beyaz erkek tavşanda karın duvarından 1,5X4cm.lik periton disseke edilerek 4 cm. uzunluğunda 6 Fr'lik polietilen tüp etrafına sarılmış 6/0 Vicryl ile sütüre edilmiş ve daha sonra peritona gömülerek karına bırakılmıştır. Bir aylık izlem süresi sonunda karın tekrar açılarak omentum ile birlikte tüp çıkarılmış ve histopatolojik incelemeye alınmıştır. Histopatolojik inceleme sonunda tüm tüplerde ortada lümen olduğu ve lümen çevresinde lenfosit, histiosit ve fiboblastlardan oluşan hücre sıralanması olduğu gözlenmiştir. Tüm tüplerde greftin bütün tabakalarının iyi damarlanmış olduğu saptanmıştır. Tüplerin yedi tanesinde çevre dokuda minimal veya orta derecede kullanılan dikiş materyaline bağlı olarak yabancı cisim reaksiyonu, ayrıca çevre dokuda yedi tüpte minimal, iki tüpte orta ve bir tüpte ise şiddetli derecede yangısal yanıt saptanmıştır. Sonuç olarak, lümen içeren ve omentumdan damarlanması ve beslenmesi iyi olan peritoneal tüp greftin kolay uygulanabilir olması, diğer ameliyat yöntemlerine göre komplikasyonsuz olması nedeniyle uzun segment üreter defektlerinde üreteral devamlılığın sağlanması amacıyla kullanılabileceği düşünülmüştür
SUMMARY: CREATING OF PERITONEUM TUBE FOR USE IN LONG SEGMENTED URETERAL DEFECTS In our study, which is a preliminary study, it was aimed to create a tube from the peritoneum by using the omentum support around a stent in order to ensure ureteral continuity. In 10 New Zeland white male rabbits, the peritoneum of 1.5X4cm was dissected from the abdominal wall and sutured with 6/0 vicryl wrapped around a 4 cm. long 6 Fr polyethylene tube, and then placed in the abdomen by being embedded in the peritoneum. At the end of one month follow-up period, the abdomen was reopened and the tube together with the omentum was removed and taken for histopathological examination. At the end of histopathological examination, it was observed that there was a lumen in the middle in all tubes, and there were cell lines consisting of lymphocytes, histiocytes and fiboblasts around the lumen. All layers of the graft were found to be well vascularized in all tubes. In seven of the tubes, foreign body reaction depending on the suture material used minimally or moderately in the surrounding tissue, also, minimal inflammatory response in seven tubes, moderate in two tubes and severe in one tube were detected in the surrounding tissue. In conclusion, it is thought that the peritoneal tube graft, which contains a lumen and has good vascularization and nutrition from the omentum, can be used to ensure ureteral continuity in long segment ureter defects due to its easy applicability and no complications compared to other surgical methods.
