The Peutz-Jeghers syndrome and its relationship with malignancy: Report of two cases

Two cases of malignancy affected with Peutz-Jeghers syndrome in the same family were described. The first patient was a 38-year old white man with a known history of 11 years of Peutz-Jeghers syndrome who was operated because of recurrent low gastrointestinal tract bleeding. Histopathologic confirmation of the resected specimen and servical mass showed Peutz-Jeghers polyps and metastasis of mucoid adenocarcinoma, respectively. The other patient was a 37-year old man and he was the brother of the patient previously described. He was diagnosed during family screening and had a history of 10 years of Peutz-Jeghers syndrome. He was admitted to the hospital because of acute bowel obstruction. At operation, intussusception with necrosis of the small bowel caused by the polyps and peritoneal secondaries were detected. Peritoneal carcinomatosis with Peutz-Jeghers polyps were confirmed by histopathologic examination. The former patient died six and the latter died seven months after the operation. Depending upon the results of the various recent reports related with Peutz-Jeghers syndrome, intensive screening with invasive treatment by using both endoscopic and surgical panpolypectomy and close clinical follow-up may be needed.
