Jüvenil romatoid artritte (JRA) gelişen disfonksiyonun, çocuk ve ailesi tarafından yapılan değerlendirmesini karşılaştırmak amacıyla yapılan bu çalışmaya, 6-17 yaşlan arasında olan JRA 'U 35 çocuk ve aileleri alındı. Ortalama hastalık süresi 4.45 yıldı. 100 sorudan oluşan bir anket hasta ve ailelerine verildi ve sonuçlar karşılaştırıldı. Bu 100 sorudan 57'si fiziksel aktivitelerle, 26'sı psikososyal fonksiyon ve diğerleri genel semptomlarla ilişkiliydi. Fiziksel aktivitelerle ilişkili olan 57 sorudan 15'i kaba motor fonksiyon, diğerleri ince motor fonksiyonun (hijyen, giyinme, beslenme ve okul) değerlendirilmesi içindi. Çocukların çeşitli işler, aktiviteler ya da durumlarda, hastalıklarının sonucu olarak karşılaştıkları güçlük ve fiziksel aktiviteleri yapabilirliğin önemi; her ifade için 6-noktalı Liken skalası ile değerlendirildi. Hasta ve ailesi arasındaki uyum düzeyini saptamak için ağırlıklı kappa skorları kullanıldı. Fiziksel fonksiyon için ortalama kappa değeri 0.615 (kaba motor için 0.62, ince motor için 0.575), psikososyal fonksiyon için 0.62, genel semptomlar için 0.72 idi. (0.6-0.8 arasındaki değerler iyi uyumu gösterir). Bu çalışmada, çocuk ve ailesi arasında disfonksiyon düzeyine ilişkin uyumun iyi olduğu saptandığından; kronik artritin, çocukların yaşam kalitesi üzerindeki etkilerinin değerlendirilmesinde çocuk ya da ailesinin kullanılabileceği sonucuna varılmıştır.
Thirtyfive children (6-17 years old) with juvenile rheumatoid arthritis and their parents were included in this study in order to compare the assesment of dysfunction in juvenile rheumatoid arthritis by the children and their families. The mean duration of the disease was 4.45 years. A questionnaire composed of 100 items was given to the patients and to their parents and the results were compared. Of the 100 items, 57 were related to physical activities, 26 to psychosocial functioning and the rest to general symptoms. Fifteen items out of the 57 which were related to physical activities were to asses the gross motor function and the rest for the f ine motor function (hygiene, wearing feeding and school). The difficulty experienced by the children in various tasks, activities or situations due to the disease as well as the importance of the capability of the physical activities were scored on a 6-points Likert scale for each item. Weighted kappa scores were used to determine the level of agreement between patients and parents. Mean kappa values for physical function was 0.615 (0.62 for gross motor, 0.575 for fine motor), 0.62 for psychosocial and 0.72 for general symptoms (the values between 0.6-0.8 indicate good agreement). It was concluded that the level'of agreement between the children and their parents was good in this study suggesting that either the parent or the child can be used for the assessment öf chronic arthritis on the quality of life of the patients.
