Hyperandrogenism: Is it a cause for missed abortion or second trimester intrauterine fetal death?

Özet
Bu çalışmanın amacı erken gebelik kayıplarında, gebe kadınlardaki hiperandrojeneminin rolünün saptanmasıdır. Missed abortus ya da intrauterin fetal ölüm saptanan 25 gebe çalışma grubunu; normal gebeliği olan 25 gebe ise kontrol grubunu oluşturdu. LH, FSH, E2, prolaktin, DHEAS, 17-hidröksiprogesteron ve serbest testosteron düzeyleri her iki hasta grubunda da ölçüldü. DHEAS ve serbest testosteron düzeyleri çalışma grubunda yüksek saptandı. Çalışma ve kontrol grubu arasındaki serum DHEAS ve serbest testosteron düzeyleri arasındaki fark istatistiki olarak anlamlı bulundu(P<0.0007 ve P<0.001). Hiperandrojeneminin, missed abortusların ve intrauterin fetal ölümlerin etiyolojisinde yeri olduğu ve infertilitedeki ovulasyon üzerine bilinen etkisinin yanında gebelikte fetal kayıplardan da sorumlu tutulabileceği kanaatine varıldı.
OBJECTIVE: To determine the association between hyperandrogenism and early pregnancy loss either in the form of missed abortion or intrauterine fetal death STUDY DESIGN: Twenty-five pregnant women with the diagnosis of missed abortion or intrauterine fetal death comprised the study group, and twenty-five pregnant women with normal pregnancy comprised the control group. LH, FSH, E2, prolactin, DHEAS, 17-hydroxyprogesterone and free testosterone levels were measured. Student's t- test was used to compare the means of hormone values, ages and duration of pregnancies. RESULTS: DHEAS and free testosterone serum levels were found to be higher in the study group. the difference between the serum levels of DHEAS and free testosterone between the study and control group was found to be statistically significant (PO.0007 and P<0.001, respectively). CONCLUSION: Hyperandrogenism might be a cause of missed abortion and intrauterin fetal death.
