Effects of iron supplementation during pregnancy on perinatal outcome

Bizim amacımız gebelikte demir desteğinin etkilerini ilk antenatal visitte ve doğumdan hemen önce hemoglobin konsantrasyonu, gebelik sırasında annenin kilo alımını, bebeğin kilosunu ve perinatal sonuçları araştırmaktır. Gebelik sırasında ileri derecede anemik olmayanlarda demir alımı gereklimidir sorusunu araştırdık. Orta derecede anemik veya hemoglobin konsantrasyonu düşük fakat normal sınırlarda gebe bir kadına demir verilmesi demir durumunu düzeltmekte , kilo alımında yada doğum zamanında demir desteğinin bariz olarak etkin olduğunu saptamadık. Bizim gözlemlediğimiz tek belirgin etki doğum ağırlığının iyileşmesi olmuştur.Her iki grupta da doğum ağırlığı karsılaştırılabilir oranlardadır. 1. dakikada bütün fetuslarda apgar skorları 7'den fazlaydı. Bununla birlikte hala daha büyük populasyonların araştırılması gerektiğine inanmaktayız.
OBJECTIVE: Our aim was to investigate the effects of iron supplementation during pregnancy on some parameters such as the hemoglobin concentration at the first antenatal visit and shortly prior to delivery, maternal weight gain during pregnancy, birth weight and perinatal outcome. Are iron supplements during pregnancy necessary? STUDY DESING: in a prospective case-control study, 180 pregnant women attending the Ege University Medical Faculty Obstetric Outpatient Clinic for antenatal care and who delivered within the period 2000-2001 were included in the study. They were divided into two groups: Both groups were randomly chosen without considering the iron status of the pregnant women and matched with control patients of similar qualities. in this study we measured hemoglobülin concentration of both groups in the first polyclinic visit and the hemoglobulin concentration shortly prior to delivery. We also measured weight gain of the mother during pregnancy, birth weight and health status of the fetus according to the apgar scores, and also time of the delivery. Student-t test and chi-square tests were used to analyze the data. RESULTS: in both groups, changes in hemoglobin concentrations at the first antenatal visit and shortly prior to delivery were not found to be significant. 87 pregnant women were given 80 mg iron (ferrous sulphate) daily throughout the pregnancy and 93 pregnant women were not given iron supplementation as a control group. the majority of the patients 'hemoglobulin concentrations were low but within normal limits in the groups. in the iron supplemented group, the mean hemoglobin concentration was 11.2 g/dl. in the control group, it was 11.97 g/dl. in the iron supplemented group, the average gestational age in the first antenatal visit was 24-25 weeks, in the control group it was 21-22 weeks. in the iron supplemented group, the mean weight gain of the mothers during pregnancy was 11.9 kg, in the control group, it was 11.5 kg. in the iron supplemented group, the mean birth weight of the fetuses were 3352.6 gr. in the control group, it was 3164.3 gr. the perinatal outcome were comparable among the two groups. in this study, we accepted that the fetus born within the period 38 to 42 weeks as term. in the iron supplemented group, there were only 3 preterm deliveries. in the non-iron supplemented group, there were 10 preterm deliveries. CONCLUSION: Iron supplementation to pregnant women who were mildly anemic or whose hemoglobulin concentrations were low but within normal limits has no effects on correcting the iron status, weight gain of the pregnant women, or time of delivery. the only significant effect we observed was improvement of birth weight of the fetus. the perinatal outcomes were comparable within both groups, the apgar scores of all fetuses were higher than 7 at the first minute. We think that still larger populations are required to evaluate this condition
