Paramedik öğrencilerinde crush sendromu bilgi düzeyini arttırmaya yönelik eğitim programı geliştirilmesi

Paramedik Öğrencilerinde Crush Sendromu Bilgi Düzeyini Arttırmaya Yönelik Eğitim Programı Geliştirilmesi Amaç: Bu çalışmada HÖASH çalışanlarının Crush Sendromu bilgi düzeylerinin değerlendirilmesinde kullanılacak bir ölçeğin geliştirilmesi ve ölçeğin uygulanması ile elde edilecek verilerden paramedik eğitimine yönelik örnek bir Crush Sendromu eğitim programının hazırlanması amaçlanmıştır. Gereç ve Yöntem: İki aşamada gerçekleştirilen çalışmanın birinci aşamasında, ölçeğin geliştirilmesi için nicel araştırma yöntemlerinden betimsel tarama kullanılmıştır. Evren, Türkiye üniversiteleri paramedik programlarında öğrenim gören mezuniyet aşamasına gelmiş öğrencilerden (n=8492) oluşmuş, çalışmaya 424 kişi katılmıştır. Ölçme araçlarından elde edilen veriler kullanılarak korelasyon matrisi AMOS 24.0.0 programına eklenmiş ve değerlendirilmiştir. Ölçek 32 sorudan ve 4 alt boyuttan oluşmuştur. İkinci aşamada eğitim programının geliştirilmesi, randomize kontrollü deneysel çalışma olarak gerçekleştirilmiştir. Geliştirilen eğitim programının etkinliği ön test ve son testlerle değerlendirilmiş, eğitime katılan öğrencilerin görüşleri eğitim değerlendirme anketi ile alınmıştır. Bulgular: Çalışma kapsamında geliştirilen Hastane Öncesi Crush Sendromu Bilgi Düzeyi Ölçeği'nin genel bilgi, tedavi ve transport alt boyutlarında yüksek derecede, ileri seviye bilgi alt boyutunda ise oldukça iyi düzeyde güvenilirliğe sahip olduğu belirlenmiştir. Katılımcıların hastane öncesi Crush Sendromu bilgi düzeylerinin, transport alt boyutunda orta düzey yeterli iken ölçeğin tümü ve diğer alt boyutlarda (genel bilgi, tedavi ve ileri düzey bilgi) yetersiz olduğu görülmüştür. Ölçekten elde edilen veriler doğrultusunda geliştirilen örnek eğitim programının uygulandığı eğitim grubunun, mevcut öğretim programına bağlı kalınarak eğitim verilen kontrol grubuna oranla başarı düzeylerinin daha yüksek olduğu belirlenmiştir. Buna göre eğitim grubunda %64 oranında başarı artışı gözlemlenirken kontrol grubunda bu artış sadece %27 oranında olmuştur. Öğrencilerin, örnek eğitim programına yönelik yüksek düzeyde olumlu düşüncelere sahip oldukları belirlenmiştir. Sonuç: Hastane Öncesi Crush Sendromu Bilgi Düzeyi Ölçeği'nin geçerli ve güvenilir bir ölçme aracı olduğu söylenebilir. Ölçekten elde edilen verilerden Türkiye'de hastane öncesi acil sağlık çalışanlarının Crush Sendromu bilgi durumlarının yeterli düzeyde olmadığı görülmüştür. Çalışma kapsamında geliştirilen örnek eğitim programının, mevcutta uygulanan eğitime göre daha etkin olduğu ve öğrencilerin büyük çoğunluğu tarafından olumlu değerlendirildiği görülmüştür. Developing a Training Program to Increase the Knowledge of Crush Syndrome in Paramedic Students Objective: In this study, it is aimed to develop a scale that will be used to evaluate the knowledge level of Crush Syndrome of pre-hospital emergency health care workers and to prepare an exemplary Crush Syndrome training program for paramedic education from the data to be obtained by applying the scale. Materials and Methods: In the first stage of the study, which was carried out in two stages, descriptive survey, one of the quantitative research methods, was used to develop the scale. The universe consisted of senior students (n=8492) studying in paramedic programs of Turkish universities, and 424 people participated in the study. Using the data obtained from the measurement tools, the correlation matrix was added to the AMOS 24.0.0 program and evaluated. The scale consists of 32 questions and 4 sub-dimensions. In the second stage, the development of the training program was carried out as a randomized controlled experimental study. The effectiveness of the developed training program was evaluated with pre-test and post-tests, and the opinions of the students who participated in the training were taken with the training evaluation questionnaire. Results: It was determined that the Prehospital Crush Syndrome Knowledge Level Scale developed within the scope of the study had a high level of reliability in the general knowledge, treatment and transport sub-dimensions, and a very good level of reliability in the advanced knowledge sub-dimension. While the prehospital Crush Syndrome knowledge level of the participants was moderate enough in the transport sub-dimension, it was observed that the whole scale and other sub-dimensions (general knowledge, treatment and advanced knowledge) were insufficient. It was determined that the training group, in which the sample training program developed in line with the data obtained from the scale, was applied had higher levels of success compared to the control group, which was trained by adhering to the current teaching program. Accordingly, while a 64% increase in success was observed in the training group, this increase was only 27% in the control group. It was determined that the students had a high level of positive thoughts towards the exemplary education program. Conclusion: It can be said that Prehospital Crush Syndrome Knowledge Level Scale is a valid and reliable measurement tool. From the data obtained from the scale, it was seen that the knowledge of Crush Syndrome of pre-hospital emergency health workers in Turkey was not at a sufficient level. It was seen that the sample education program developed within the scope of the study was more effective than the current education and was positively evaluated by the majority of the students.
