Özet
Perlit, yüksek oranda hava içermesi nedeniyle yalıtım ve süzme gibi alanlarda kullanılan, toz şeklinde doğal bir volkanik camdır, izmir Cumaovası'nda perlit işleyen bir fabrikanın 99 işçisi anamnez, fizik muayene, akciğer grafisi, solunum fonksiyon testleri (SFT) ve difüzyon kapasitesi (DLCO) ölçümleri ile değerlendirilmiş, DLCO < %80 olan 22 işçiye bronkoalveoler lavaj (BAL) yapılmıştır. Ortalama yaş 45 (± 4.4) yıl, çalışma süresi 16.6 (± 6.6) yıldır. Tümü sigara içicisi olan işçilerin ortalama 22.1 (± 9.3) paket/yıl sigara içtikleri saptanmıştır. SFT'de FEV1 %90.9 (± 14.4), FVC %91.3 (± 19.8), FEV1/ FVC %101.9 (± 10.9), DLCO %65.3 (± 11.1), difüzyon kapasitesi/alveol ventilasyonu (DLCO/VA) %58.6 (± 21.8) olarak bulunmuştur. BAL sıvısında makrofaj %77.8 (± 11.6), lenfosit %15.6 (± 10.9), nötrofil %5.2 (± 2.7), eozinofil %1.3 (± 0.7) oranında saptanmıştır. BAL'daki nötrofil artışında sigara içiminin katkısı olmakla birlikte, DLCO < %80 olan işçilerin çalışmaya alınmış olması nedeniyle mesleki perlit maruziyetinin değerlendirilmesinde BAL'da nötrofil artışının dikkate alınması yararlı olacaktır.
Perlit is a volcanic glass which is in the form of dust and it contains air in high proportion therefore it is being used in fields like filtering and insulation. The 99 workers of a factory which is processing perlit in Izmir Cumaovasi are evaluated by way of medical history, clinical examination, chest radiograph, pulmonary function tests and DLCO. The 22 workers who had DLCO < 80% are made bronchoalveolar lavage. Mean age (± SD) and mean number (± SD) of years at work among 22 workers were 45 (± 4.4) years and 16.6 (± 6.6) years respectively. All the workers were smokers and they had a cigarette smoking history of a mean 22.1 (± 9.3) pack-years. The workers had the following mean values: FEV-1 90.9% (± 14.4), FVC 91.3% (± 19.8), FEV1/FVC 101.9% (± 10.9), DLCO 65.3% (± 11.1), DLCO/VA 58.6% (± 21.8); mean percent of alveolar macrophages in BAL fluid 77.8% (± 11.6), lymphocytes 15.6% (± 10.9), neutrophils 5.2% (± 2.7) and eosinophils 1.3% (± 0.7). Although smoking has contributed to the increase of neutrophils in BAL fluid, it will be of great use to consider that neutrophil increase in BAL in assessment occupational perlit exposure since the workers with DLCO < 80% has been included in the related study.
