Özet

This study aimed to determine the effects of hydroxyethyl starch (HES) or gelatine solutions on hemodynamics and postoperative bleeding, when used for acute isovolemic hemodilution (AIH) during coronary artery bypass grafting (CABG) surgery. Eighty patients had fentanyl infusion, combined with inhalation anesthesia and monitoring with ECG, central temperature, radial, pulmonary arterial and central venous pressures was performed. Cardiac output (CO) was measured and systemic vascular resistance (SVR), pulmonary vascular resistance (PVR), oxygen delivery (DO2) and oxygen consumption (VO2) were calculated. Hemodilution was performed using HES (Group I, n=40) or gelatine (Group II, n=40). All patients had AIH to the extent possible without reaching hematocrit (Hct)<30 %. Hemodynamic measurements performed before and after AIH were recorded together with coagulation tests and postoperative mediastinal drainage volume. Perioperative last Hct, amount of transfused blood, mediastinal drainage, pre-AIH and post-AIH Hct, mean arterial pressure (MAP), CO, SVR, PVR, DO2 and VO2 values were similar between groups (p>0.05). HES or gelatine solutions have similar effects on hemodynamics and postoperative bleeding, when used at a mean volume of 10 mL kg-1 for AIH during CABG surgery.
