Son iki haftadır devam eden ateş ve son üç gündür devam eden dudaklarındaki çatlaklardan kanama nedeniyle başvuran üç yaşındaki erkek hastanın serum transaminazlarında artış ile birlikte koagülasyon testlerinde bozulma dikkati çekti. Gruber-Widal testi ve serum anti-HAV IgM pozitif bulunan, kan kültüründe Salmonella typhi üreyen olgu tifo-hepatit A ko-infeksiyonu tanısı aldı. Olgu, çocuk yaş grubunda bu ko-infeksiyonun daha önceden bildirilmemiş olması nedeniyle sunuldu. Tifoda hafif derecede transaminaz artışı sık görülmekle birlikte, belirgin transaminaz artışı ve kanama eğilimi varlığında tifo-hepatit A ko-infeksiyonu olabileceği düşünülmelidir.
A 3 year-old boy, with persistent fever for 2 weeks before admission concomitant with mild bleeding from fissures over his lips for the last 3 days was admitted to the hospital. The patient had deteriorated coagulation tests in addition to elevated serum transaminase levels. Investigations revealed that the patient had both thyphoid fever and acute viral hepatitis A. This co-infection in childhood has not been previously reported. Although the liver is commonly involved in thyphoid fever, a concomitant infection of hepatitis A should also be considered in cases with thyphoid fever having biochemical findings of hepatic involvement, especially in those having abnormal coagulation test results.
