Özet
Amaç: Bu tez çalışmasında amaç; atriyal fibrilasyon (AF) tanılı hastalara acil servisten taburculuk sırasında verilen eğitimin etkinliğini değerlendirmekti. Yöntem: Girişimsel, ön-test ve son-test modeli ile yürütülen randomize kontrollü çalışmaya Mart-Eylül 2023 tarihleri arasında Ege Üniversitesi Sağlık Uygulama ve Araştırma Merkezi acil servisine başvuran ölçütleri karşılayan 208 hasta dahil edildi. Hastalar randomize olarak girişim ve kontrol gruplarına atandı. Veri toplama araçları olarak; tanıtıcı özelliklerin ve hastalığa ilişkin soruların yer aldığı 14 soruluk "Hasta Tanıtım Formu", "Atriyal Fibrilasyon Eğitim Kitapçığı (Girişim Grubu)", AF ile ilgili genel bilgilerin, K vitamini antagonisti (warfarin) ile ilgili bilgilerin ve yeni nesil oral antikoagülanlar (YOAK) ile ilgili bilgilerin sorgulandığı 23 maddelik " Atriyal Fibrilasyon Bilgi Ölçeği", AF'nin yaşam kalitesini nasıl etkilediğini belirlemeye yönelik soruların yer aldığı "Atriyal Fibrilasyonun Yaşam Kalitesi Üzerine Etkisi Anketi", "Morbidite ve Mortalite Değerlendirme Formu" ve yapılan telefon görüşmelerini standardize etmek amacıyla hazırlanmış "Yapılandırılmış Telefon Görüşmesi Formu" kullanıldı. Verilerin istatistiksel değerlendirmesinde; tanımlayıcı istatistiksel yöntemler, niceliksel verilerin karşılaştırılmasında, iki bağımsız grup karşılaştırması için bağımsız örneklem t testi, bağımlı gruplar arasındaki fark için ise bağımlı örneklem t testi yapıldı. Bağımsız kategoriler arasındaki ilişki ki kare analizi ile incelenirken bağımlı kategoriler için Mc Nemar testi kullanıldı. Normal dağılım olmayan bağımsız kategoriler için Mann Whitney U testi gerçekleştirildi. İstatistiksel anlamlılık düzeyi p<0,05 olarak alındı. Bulgular: Hastaların % 49,5'ini kadın, % 50,5'ini erkekler oluşturdu. Girişim grubundaki katılımcıların yaş ortalaması 59,94±8,60; kontrol grubundaki katılımcıların yaş ortalaması 55,93±10,5 olarak hesaplandı. AF bilgi düzeyi puanlarının gruplar arasındaki karşılaştırması incelendiğinde; son test AF bilgi düzeyi için girişim ve kontrol grubu arasında istatistiksel olarak anlamlı bir farklılık olduğu ve girişim grubu bilgi düzeyinin kontrol grubu bilgi düzeyinden daha yüksek olduğu belirlendi (p<0,05). Atriyal Fibrilasyonun Yaşam Kalitesi Üzerine Etkisi Anketi puanlarının son test değerleri için girişim ve kontrol grubu arasında istatistiksel olarak anlamlı bir farklılık olduğu ve girişim grubu değerlerinin kontrol grubu değerlerinden daha düşük olduğu tespit edildi (p<0,05). Sonuç: Acil servisten taburculuk sırasında verilen eğitimle AF'li hastaların hastalıklarıyla ilgili bilgi düzeylerinin yükseldiği, buna bağlı yaşam kalitesinin arttığı, acil servise başvuru sayılarının azaldığı ve daha iyi komplikasyon yönetiminin sağlandığı görüldü. Acil servise en sık başvuran hasta gruplarından olan AF tanılı hastalara verilen eğitimlerin yaygınlaştırılmasının hastalık yönetimini ve hastaların özgüvenini olumlu yönde etkileyeceği düşünülmektedir. Method: 208 patients who met the criteria and were admitted to the Ege University Health Application and Research Center emergency department between March 2023 and September 2023 were included in the randomized controlled study conducted with an interventional, pre-test and post-test model. Patients were randomly assigned to intervention and control groups. As data collection tools; "Patient Introduction Form" with 14 questions including introductory features and questions about the disease, "Atrial Fibrillation Training Booklet (Initiative Group)", general information about AF, information about vitamin K antagonist (warfarin) and new generation oral anticoagulants (NOAC). The 23-item "Atrial Fibrillation Information Scale", which questions information about AF, the "Impact of Atrial Fibrillation on Quality of Life Questionnaire", which includes questions to determine how AF affects the quality of life, the "Morbidity and Mortality Evaluation Form", and was prepared to standardize telephone interviews. "Structured Telephone Interview Form" was used. In the statistical evaluation of the data; Descriptive statistical methods were used to compare quantitative data, independent sample t test was used for comparison of two independent groups, and dependent sample t test was used for the difference between dependent groups. While the relationship between independent categories was examined with chi-square analysis, Mc Nemar test was used for dependent categories. Mann Whitney U test was performed for independent categories that were not normally distributed. Statistical significance level was taken as p<0.05. Results: 49.5 % of the patients were women and 50.5 % were men. The average age of participants in the intervention group was 59.94±8.60; The average age of the participants in the control group was calculated as 55.93±10.5. When the comparison of AF knowledge level scores between groups is examined; It was determined that there was a statistically significant difference between the intervention and control groups for the post-test AF knowledge level and the knowledge level of the intervention group was higher than the knowledge level of the control group (p<0.05). It was determined that there was a statistically significant difference between the intervention and control groups for the post-test values of the Impact of Atrial Fibrillation on Quality of Life Questionnaire scores and that the intervention group values were lower than the control group values (p<0.05). Conclusion: It was observed that the education given at the time of discharge from the emergency department increased the knowledge level of patients with AF about their disease, increased their quality of life, decreased the number of admissions to the emergency department, and provided better complication management. It is thought that disseminating the training given to patients diagnosed with AF, which is one of the patient groups most frequently admitted to the emergency department, will positively affect disease management and patients' self-confidence.
