Özet
Spinal füzyon, spinal cerrahinin en sık uygulanan bir
işlemi olmasına rağmen özellikleri halen tam olarak
bilinmemektedir. Yüksek devirli drill bu işlemdeki rolü ise
tartışmalıdır. Bu çalışmada dekortikasyonun spinal füzyondaki etkisi araştırıldı. 28 yetişkin Yeni Zelanda
beyaz tavşan kullanılarak in vivo deneysel hayvan modeli
oluşturuldu. Tüm hayvanlarda L2-L6 arası omurga, iki
traftan subperiosteal diseksiyon ile ortaya çıkarıldı.
Hayvanlar uygulanan işlemlere göre üç gruba ayrıldı:
Yüksek devirli dril ile dekortikasyon+otogreft (grup 1), 15
nolu bistrü ile dekortikasyon+
otogreft (grup 2) ve dekortikasyon yapılmadan+otogreft
(grup 3). Cerrahiden sonra altıncı haftada hayvanlar
öldürülerek oluşan füzyon kütlesi makroskobik olarak, elle
stres testi ile,
Sonuçlar: Segmentlere göre değerlendirmede solid veya
nonfuzyon olarak elde edildi.
Although spinal fusion is one of the most commonly used procedures in spinal surgery its properties are not fully known. The role of high speed drill is stil not clear. In this animal model, we investigated the effect of high speed drill on spinal fusion. The study included 28 adult New Zeland white rabbits underwent bilateral posterolateral spinal fusion at L5-L6 using autogenous iliac bone graft. The animals were assigned to three groups. group 1, high speed dril using for decortication, and autografting group 2, surgical knife using for decortication and autografting group 3, without decortication only autografting. All animals were sacrificed in the sixth postoperative week. The fusion mass was assessed by means of macroscopic evaluation, manual stress test, Results: Based on segmentary evaluations, fusion was obtained in solid or non solid groups 1, 2, and 3, respectively. Conclusion: The results show that decortication should be an essential step for spinal fusion if spinal instrumentation is not considered. However, in case of a high speed drill with decortication seems to exert no positive effect on the formation of spinal fusion.
