Özet
Bu çalışma Ege Üniversitesi KBB Anabilim Dalı'nda Ocak 1993 - Nisan 1995 tarihleri arasında larengeal kanser nedeniyle parsiyel veya rekonstrüktif larenjektomi uygulanan 50 vaka üzerinde gerçekleştirilmiştir. Vakalara postope-ratuar dönemde yutma reedükasyonu uygulanarak bunun fonksiyonel sonuçları, yaş, operasyon tipi, çıkarılan yapılar gibi faktörlerin yutmaya etkileri ve yutma reedükasyonu sırasında karşılaşılan sorunlar ve çözüm yolları araştırılmıştır. Olguların tümü reedükasyon sonrasında aspirasyonsuz ağızdan beslenme yapabilmeyi öğrenebilmişlerdir. Operasyonun tipi readaptasyon süresini önemli ölçüde etkilerken (p<0.05), yaş etkili bir faktör olarak bulunmamıştır (p>0.05). Yutma reedükasyonunun tüm parsiyel ve rekonstrüktif larenjektomi olgularına uygulanması gereken, ameliyat öncesi ve sonrası planlı, organize ve disiplinli bir çalışmayı gerektiren etkili ve başarılı bir yöntem olduğu sonucuna varılmıştır.
This study has been performed on a group of 50 patients with laryngeal cancer who underwent either partial or reconstructive laryngectomies in the ENT Department of Ege University, Medical Faculty between January 1993 and April 1995. The patients had deglutitional training in the postoperative period. Functional results of training, the effects of factors on deglutition such as age of the patient, the type of the operation and the structures excised, were discussed as well as the problems encountered during deglutitional training and their solutions. All the patients succeeded in oral feeding by the end of the training period. The type of the operation was found to be an important factor on readaptation period (p<0.05) Whereas age was not (p>0.05). Each patient who underwent either partial or reconstructive laryngectomy should have deglutitional training. This training seems to be an effective and successful method requiring a well-organized work that must be planned both pre- and postoperatively.
