The influence of chlamydial infections on pregnancy rates and prognosis in ART treatment cycles

Clamidya enfeksiyonuna sekonder olarak infertilite gelişebileceği uzun zamandan beri bilinmektedir. Son yıllarda genital clamidya enfeksiyonunda belirgin bir artış gözlenmektedir. Enfeksiyonların çoğu zaman sinsi seyrettiği, tubal oklüzyonlara, peritubal adezyonlara, ayrıca kronik endorhetrite yol açarak ortaya çıkan konsepsiyon kusuru nedeniyle hem doğal yoldan hem de ART sikluslarında başarı oranlarını düşürdüğü bildirilmektedir. Çalışmamızda son 5 yıl içerisinde IVF-ET Merkezimize başvuran 651 olgu dosyası retrospektif olarak incelendi. Clamidya enfeksiyonu pozitif saptanan 114 olgudan ART uygulaması yapılan 49 tanesinde gebelik oranlan, oluşan gebeliklerin prognozu, kontrol grubu olarak alınan 49 kişilik 2. Grup ile karşılaştırıldı. Clamidya pozitif olanlarda gebelik oranı %22.4 iken, clamidya negatif olanlarda bu oran %28.6 olarak saptandı. Aradaki fark istatistik açıdan anlamlı değildi(p>0.05). Gebeliklerin prognozu incelendiğinde ise clamidya pozitif olanlarda terme ulaşan gebelik oranı %54.5 iken clamidya negatif olanlarda bu oran %92.9 idi. Aradaki fark istatistiksel olarak anlamlı bulundu.(p<0.05).
OBJECTIVE: To determine the effect of chlamydial infections on pregnancy rates and prognosis in ART treatment cycles. STUDY DESIGN: We have examined the records of 651 women who have attended our IVF-ET center during the last 5 years retrospectively. Out of 114 women where chlamydial infection was found as positive, 49 were treated by ART. These ART cycles were compared with 49 control group cycles for pregnancy rates and the prognosis of these pregnancies. RESULTS: While 22.4% of pregnancy rate was found in chlamydial infection positive women, 28.6% of pregnancy rate was found for chlamydia negative women, although this difference was not statistically significant (p>0.05). On the other hand only 54.5% of chlamydia positive women delivered where as 92.9% of chlamydia negative women delivered. This difference was statistically significant(p<0.05). CONCLUSION: An increase in chlamydial infections has been observed during the last years.Due to the silent clinical state of the infections, tubal, peritubal adhesions also chronic endometritis may lead to implantation failure thus to decreasing rates of pregnancy both in natural and ART cycles.
