Avrupa Birliği'ne üyelik sürecinde hasta güvenliği kapsamında kurumsal kapasitenin geliştirilmesi

Araştırma, Avrupa Birliği' ne üyelik sürecinde hasta güvenliği kapsamında kurumsal kapasitenin geliştirilmesi amacıyla yürütülmüş metodolojik ve yarı deneysel bir araştırmadır. Araştırmanın; Hasta Güvenliği Ekseninde Sağlık Kurumlarının Kurumsal Kapasitelerinin Avrupa Birliği Sağlık Politikaları Kapsamında Değerlendirilmesi (HG-ABSK) Ölçeği' nin geliştirilmesi aşaması İzmir İlindeki; 4 (dört) kamu hastanesinde yürütülmüştür. HG-ABSK Ölçeği' nin geliştirilmesi aşamasında cerrahi birimlerde (ameliyathane, yoğun bakım, acil, cerrahi klinikler) çalışan 627 sağlık çalışanı (hekim, hemşire, sağlık teknisyeni/teknikeri) örneklem grubunu oluşturmuştur. Avrupa Birliği' ne Üyelik Sürecinde Hasta Güvenliği Kapsamında Kurumsal Kapasitenin Geliştirilmesi Eğitimi "nin uygulanması aşamasında cerrahi birimlerde yönetici pozisyonundaki 40 sağlık çalışanı (hekim, hemşire, sağlık teknisyeni/teknikeri) örneklem grubunu oluşturmuştur. Verilerin analizinde SPSS 25.0 ve AMOS programları kullanılmıştır. Sayı, yüzde, ortalama, standart sapma gibi tanımlayıcı istatistik metodları verileri değerlendirmede kullanılmıştır. Ölçeğe ilişkin güvenirlikler Cronbach Alpha ile analiz edilmiştir. Ölçeğe ilişkin yapı geçerliliği analizleri için Açıklayıcı Faktör Analizi ve Doğrulayıcı Analizi kullanılmıştır. Maddelerin ayırt edicilik güçlerinin incelenmesinde %27 alt üst madde analizi kullanılmıştır. Mann Whitney U testi iki bağımsız grup arasındaki fark için, ikiden fazla bağımsız grup karşılaştırılmasında ise Kruskal Wallis H testi uygulanmıştır. Pilot uygulama ile ilgili ölçeğe ilişkin güvenirlikler cronbach alpha ve iki yarı güvenirliği ile incelenmiştir. Ölçeğin kapsam geçerliliği Lawshe Tekniği' ne göre değerlendirilmiştir. HG-ABSK Ölçeği, dört boyut ve 36 maddeden oluşmaktadır. HG-ABSK Ölçeği tanımlayıcı istatistikleri incelendiğinde ölçek geneli için puan ortalamaları 3,66±0,43; Avrupa Birliği' ne Üyelik Sürecinde Hasta Güvenliği Kapsamında Kurumsal Kapasitenin Geliştirilmesi Eğitimi' ne katılan sağlık çalışanlarının ön ve son testleri ölçek toplam puanları, F1, F2, F3, F4 alt boyut puanları arasında istatistiksel olarak anlamlı bir fark olduğu görülmektedir (p<0,05). Araştırma sonucunda, HG-ABSK Ölçeği' nin geçerli ve güvenilir bir ölçek olduğu ve eğitim programının AB' ye üyelik sürecinde hasta güvenliği kapsamında kurumsal kapasitenin gelişimde etkili olduğu bulunmuştur.

The research is a methodological and quasi-experimental research conducted to improve institutional capacity within the scope of patient safety in the European Union membership process. The development phase of the research; Evaluation of Institutional Capacity of Health Institutions in the Axis of Patient Safety within the Scope of European Union Health Policies (HG-ABSK) Scale was carried out in 4 (four) public hospitals in Izmir Province. The sample group consisted of 627 health workers (physicians, nurses, health technicians/technicians) working in surgical units (operating theatre, intensive care, emergency, surgical clinics) during the development phase of the HG-ABSK Scale. The sample group consisted of 40 health workers (physicians, nurses, health technicians/technicians) who were in managerial positions in surgical units during the implementation phase of the Training on the Development of Institutional Capacity within the Scope of Patient Safety in the European Union Membership Process. SPSS (Statistical Package for Social Sciences) for Windows 25.0 and AMOS programs were used for data analysis. Descriptive statistical methods such as number, percentage, mean, and standard deviation were used to evaluate the data. Reliabilities related to the scale were analyzed with Cronbach Alpha. 'Exploratory Factor Analysis and Confirmatory Analysis' were used to analyze the construct validity of the scale. 27% sub-item analysis was used to examine the discrimination power of the items. Mann Whitney U test was used for the difference between two independent groups and the Kruskal Wallis H test was used for the comparison of more than two independent groups. The reliability of the scale related to the pilot application was analyzed with Cronbach Alpha two-half liability. The content validity of the scale was evaluated according to the Lawshe technique. HG-ABSK Scale consists of four dimensions and 36 items. When the descriptive statistics of the HG-ABSK Scale were analyzed, the mean score for the overall scale was 3.66±0.43; It is seen that there is a statistically significant difference between the pre and post-test scale total scores, F1, F2, F3, F4 sub-dimension scores of the healthcare professionals who participated in the Training on the Development of Institutional Capacity within the Scope of Patient Safety in the European Union Membership Process (p<0.05). As a result of the study, it was found that the HG-ABSK Scale is a valid and reliable scale and the training program is effective in the development of institutional capacity within the scope of patient safety in the EU membership process.
