Özet
Amaç: Araştırma depresyon tanılı hastalara uygulanan logoterapi temelli müdahalenin depresif belirtiler, psikolojik acı ve yaşamın anlamına etkisini incelemek amacıyla yürütülmüştür. Yöntem: Araştırma ön test son test izlem ölçümlü, kontrol gruplu, yarı deneysel araştırma tasarımındadır. Örneklem seçimi iki aşamada gerçekleştirilmiş olup, ilk aşamada Eylül 2020-Eylül 2022 tarihleri arasında depresyon tanısı almış 48 hasta araştırmacıya yönlendirilmiştir. Hastalar sırasıyla girişim ve kontrol gruplarına atanmıştır. Girişim grubunda 12, kontrol grubunda 16 olmak üzere toplam 28 hasta ile araştırma tamamlanmıştır. Araştırmada veri toplama araçları olarak "Tanıtıcı Bilgi Formu", "Beck Depresyon Envanteri", "Psikolojik Acı Ölçeği" ve "Yaşamın Anlamı Ölçeği" kullanılmıştır. Girişim grubundaki hastalara çevrim içi (online) logoterapi temelli müdahale haftada bir oturum olacak şekilde toplam sekiz oturum olarak uygulanmıştır. Logoterapi temelli müdahalenin etkisini değerlendirmek için girişim ve kontrol grubundaki tüm hastalara zaman serisi noktalarında (ön test, son test, 3.ay izlem test) Beck Depresyon Envanteri, Psikolojik Acı Ölçeği ve Yaşamın Anlamı Ölçeği uygulanmıştır. Verilerin analizinde tanımlayıcı istatistikler, bağımsız t testi, ki kare analizi, tekrarlı ölçümlerde varyans analizi, Bonferroni düzeltmesi uygulanmıştır. Bulgular: Araştırma sonuçları zaman serisi içerisinde gruplar bazında değerlendirildiğinde, girişim grubunda Beck Depresyon Envanteri toplam puan ortalamalarında (F=66.199, p<0.01) kontrol grubuna oranla istatistiksel olarak anlamlı şekilde düşüş saptanmıştır. Grupxzaman etkileşimi açısından Beck Depresyon Envanteri etki büyüklüğü 0.718 olarak belirlenmiştir. Girişim grubu Psikolojik Acı Ölçeği toplam puan ortalamalarında (F=66.494, p<0.01) kontrol grubuna oranla istatistiksel olarak anlamlı şekilde düşüş saptanmıştır. Grupxzaman etkileşimi açısından Psikolojik Acı Ölçeği etki büyüklüğü 0.719 olarak belirlenmiştir. Girişim grubu Yaşamın Anlamı Ölçeği toplam puan ortalamalarında (F=12.453, p<0.01) kontrol grubuna oranla istatistiksel olarak anlamlı şekilde artmıştır. Grupxzaman etkileşimi açısından Yaşamın Anlamı Ölçeği etki büyüklüğü 0.324 olarak belirlenmiştir. Yaşamın Anlamı Ölçeği- Var Olan Anlam Alt Boyutunda gruplar arası farklılığın istatistiksel olarak anlamlı olmadığı, zaman serisinde gruplar arası farkın girişim grubu lehine anlamlı olduğu bulunmuştur (F=13.183, p<0.01). Grupxzaman etkileşimi açısından Yaşamın Anlamı Ölçeği- Var Olan Anlam Alt Boyutu etki büyüklüğü 0.336 olarak belirlenmiştir. Yaşamın Anlamı Ölçeği- Bulunmaya Çalışılan Anlam Alt Boyutunda gruplar arası farklılığın istatistiksel olarak anlamlı olmadığı, zaman serisinde gruplar arası farkın girişim grubu lehine anlamlı olduğu saptanmıştır (F=7.075, p<0.01). Grupxzaman etkileşimi açısından Yaşamın Anlamı Ölçeği- Bulunmaya Çalışılan Anlam Alt Boyutu etki büyüklüğü 0.214 olarak belirlenmiştir. Sonuç: Çevrim içi (online) olarak uygulanan logoterapi temelli müdahalenin, depresif belirtiler ve psikolojik acı düzeyini azaltmada etkili olduğu ve bu olumlu etkilerin uzun dönem (3 ay) izlemde devam ettiği sonucuna ulaşılmıştır. Logoterapi temelli müdahalenin bireylerin yaşamı anlamlandırma süreçlerinde uzun dönemde etkisini gösterdiği saptanmıştır. Yaşamdan anlam bulamayan, depresif belirtiler ve psikolojik acı yaşayan depresyon tanılı hastalara çevrim içi logoterapi temelli müdahale uygulanabilir. Logoterapi temelli müdahalenin farklı psikoterapötik müdahalelerle karşılaştırmalı araştırmaların yapılması literatüre katkı sağlayabilir. Aynı zamanda daha geniş örneklemde, uzun süreli izlemlerle, randomize kontrollü araştırmaların yapılması önerilmektedir.

"Descriptive Information Form", "Beck Depression Inventory", "Psychache Scale" and "Meaning of Life Questionnaire" were used as data collection tools in the study. The online logotherapy-based intervention was administered to the patients in the intervention group as a total of eight sessions, once a week. In order to evaluate the effect of the logotherapy-based intervention, the Beck Depression Inventory, the Psychological Pain Scale, and the Meaning of Life Questionnaire were administered to all patients in the intervention and control groups at time series points (pretest, posttest, follow-up test at 3rd month). In the analysis of the data, descriptive statistics, independent t-test, chi-square analysis, analysis of variance in repeated measurements, Bonferroni correction were applied. Results: When the research results were evaluated on the basis of groups within the time series, a statistically significant decrease was found in the Beck Depression Inventory total score averages (F=66.199, p<0.01) in the intervention group compared to the control group. In terms of groupxtime interaction, the effect size of Beck Depression Inventory was determined as 0.718. A statistically significant decrease was found in the intervention group's total mean score on the Psychological Pain Scale (F=66,494, p<0.01) compared to the control group. In terms of groupxtime interaction, the effect size of the Psychological Pain Scale was determined as 0.719. The intervention group had a statistically significant increase in Meaning of Life Questionnaire mean total score (F=12.453, p<0.01) compared to the control group. In terms of groupxtime interaction, the effect size of the Meaning of Life Questionnaire was determined as 0.324. It was found that the difference between the groups in the Meaning of Life Scale-Existing Meaning in Life Sub-Dimension was not statistically significant, and the difference between the groups in the time series was significant in favor of the intervention group (F=13.183, p<0.01). In terms of groupxtime interaction, the effect size of the Meaning of Life Scale-Existing Meaning in Life Sub-dimension was determined as 0.336. The difference between the groups in the Meaning of Life Scale- Search for Meaning in Life Sub-dimension was not statistically significant, and the difference between the groups in the time series was found to be significant in favor of the intervention group (F=7.075, p<0.01). In terms of groupxtime interaction, the effect size of the Meaning of Life Scale- Search for Meaning in Life Sub-dimension was determined as 0.214. Conclusion: It has been concluded that the online logotherapy-based intervention is effective in reducing the level of depressive symptoms and psychological pain, and these positive effects continue in the long-term (3 months) follow-up. It has been determined that logotherapy-based intervention has a long-term effect on individuals' processes of making sense of life. Online logotherapy-based intervention can be practiced to patients with a diagnosis of depression who cannot find meaning in life, experience depressive symptoms and psychological pain. Comparative research of logotherapy-based intervention with different psychotherapeutic interventions can contribute to the literature. At the same time, it is recommended to conduct randomized controlled studies with a larger sample and long-term follow-up.
